Your Opportunity:

Based out of the Sheldon M Chumir Health Centre, under direction of the Manager of the Calgary STI Clinic & Safeworks Outreach, the Outreach Worker – Safeworks Outreach – provides comprehensive, individualized support, and case management for people affected by sexually transmitted and blood-borne infections (STBBI). Grounded in a mission to reduce transmission and improve health outcomes for vulnerable individuals, service delivery is highly adaptable, reaching participants over the phone, virtually, in drop-in spaces, and through direct street and community-based outreach. Daily duties include providing health promotion education, distributing disease prevention supplies, offering referrals to health and social services, assisting clients with system navigation and appointments, and delivering tailored consultations to the broader healthcare team. Collaboration is central to this position, which operates alongside the teams such as Safeworks Outreach, STI Clinic, Partner Notification, and community partners to effectively engage clients in care. Additionally, the Outreach Worker must maintain competent practice and remain accountable to the public according to the regulations and standards defined by their profession’s applicable college. This position is part of Primary Care Alberta's commitment to building an inclusive workplace; consistent with organizational equity, diversity, and inclusion initiatives, Truth and Reconciliation Calls to Action, and section 15 (2) of the Canadian Human Rights Act, preference for this role may be given to qualified candidates who identify as an Indigenous person (First Nations, Métis, and Inuit).

Description:

As an Outreach Worker, you will work as a member of a multidisciplinary team, supporting clients to stabilize in their current environments as an extension of their clinical care service. As an Outreach Worker, you will conduct outreach assessments, determine outreach service goals and develop outreach treatment plans in collaboration with clients and clinicians. You will facilitate client connections with community and government supports and advocate for clients with these external service providers. You will focus on establishing housing, financial, food and clothing, transportation and social supports for clients. You will be responsible for providing education and coaching support related to daily living skills and activities (attending appointments, money management, searching for jobs, grocery shopping, etc.), observing and reporting changes in the clients' behaviours, participation and ability to function independently to the interdisciplinary team. You will evaluate client progress and functioning and make adjustments to the outreach treatment plan as required. As a part of your role as an Outreach Worker, you may be required to provide outreach crisis stabilization support for clients, exercising various de-escalation techniques and linking clients with the appropriate supports (EMS, Police and Crisis Team, etc.). You also may be required to enter clients' residences and assist clients to transfer in and out of vehicles, care facilities and residences.

Required Qualifications:

Completion of diploma from a post-secondary program in a related field.

Additional Required Qualifications:

Minimum 1-3 years’ experience working with and creating safe spaces for diverse and marginalized populations including all sexual orientations, gender identities & expressions, those who identify as people of color, Indigenous peoples, people who use substances, unhoused people, people with mental health conditions, people with disabilities, and/or newcomers to Canada. Computer proficiency with complex programs and databases; documentation is completed in the Electronic Medical Records. Ability to manage complex clients with minimal supervision. Ability to think independently and make decisions in a fast-paced and demanding work environment. Excellent attendance record & reliability. Excellent communication skills (both written and verbal) demonstrated through previous employment records & references. Ability to independently support a patient to the ground or sustain a patient in Trendelenburg position (lift/carry 30+ lbs. - waist to floor, floor to crown). Ability to handle periods of intense activity and multi-task throughout high-volume clinics/events, including walking up to a distance of 4km per shift. Ability to sustain periods of stationary activity. Ability to independently perform the skills of a certified BLS provider. Knowledge and understanding of the Truth & Reconciliation Commission Calls to Action.

Preferred Qualifications:

Degree/diploma in social work, social sciences, or any other related field. Minimum one year within the past three years’ experience in a health care setting with a preference to those with experience in clinical settings providing STBBI support. Knowledge of STIs, HIV, and Hep C. Experience working with health professionals & the public. Experience with data entry, databases, and working with medical forms. Experience and knowledge of Indigenous communities and harm reduction.
