PRIMARY FUNCTION
The Nurse Practitioner (NP) is a Registered Nurse (RN) in the Extended Class with additional educational preparation and experience, who possesses and demonstrates the competencies to autonomously diagnose, order and interpret diagnostic tests, prescribe pharmaceuticals and perform specific procedures within their legislated scope of practice.
The NP must know and practice in accordance with the standards relevant to the practice area as well as the legislation and regulations applicable to an NP practice:
- Regulated Health Professions Act, 1991;
- Nursing Act, 1991;
- Laboratory and Specimen Collection Centre Licensing Act;
- Healing Arts Radiation Protection Act, 1990;
- Public Hospitals Act, 1990;
- Health Protection and Promotion Act, 1990.
Reporting to the Program Operational Director, the NP works in an extended nursing role demonstrating a high level of autonomy and expert skill in order to formulate clinical decisions, appropriately manage acute illness and promote wellness within their area of specialty.
The NP explicitly consults the interprofessional team where care is required outside of the NP scope of practice and/or not covered by a Medical Directive.
Within this role, the NP is accountable for contributing to the delivery of the Kingston General Hospital strategy and must demonstrate an awareness of and be responsible for actively promoting and supporting patient-and-family-centered care and engagement.
PRIMARY RESPONSIBILITIES & DUTIES:
Clinical Domain
The NP works autonomously to diagnose, order and interpret diagnostic tests, prescribe pharmaceuticals and perform procedures within the legislated scope of practice.
- Assess, analyze and synthesize a wide range of information using academic evidence and advanced clinical knowledge to develop a patient plan of care.
- Holds overall accountability for a patient’s care and participates in timely patient flow planning, preventing delays by collaborating other members of the interprofessional team and leadership as appropriate.
- Manages patients autonomously within their scope of practice and is responsible for arranging transfer of care to a physician when the patient’s care exceeds the clinical scope of the NP.
- Serves as the primary point of contact for the patient, their family, and the care team. The NP ensures continuity of care is maintained during patient transitions in care, such as discharge, and will ensure appropriate follow-up is coordinated
Leadership Domain
The NP will identify risk related to clinical practice which is managed through observation, assessment, and discussion with the involved interprofessional team members.
- Improve clinical practice through the development, implementation and evaluation of practice guidelines, standards, policies and procedures that promote excellence in nursing practice in collaboration with key stakeholders.
- Participates in clinical and organizational committees
- Provide leadership by supporting and fostering evidence-based practice and continuous quality improvement processes within the organization.
- Acts as a resource to individuals and groups within the Program and throughout the organization by promoting and ensuring excellence in nursing practice.
- Provides leadership and consultancy in professional practice to other nursing colleagues within and outside the organization locally and regionally, and to other members of the interprofessional care team.
- Advocates for individuals, families, groups and communities in relation to treatment, the health-care system and policy decision that affect health and quality of life.
- Identifies learning needs of nurses and other members of the interprofessional team and will address those needs within resources.
Research/Quality Improvement Domain
The NP will ensure that patient care and nursing practice innovations within the Program are supported by research, and are reflective of, and congruent with, evidence based or current best practice guidelines/standards of practice.
- Participates in and utilizes quality improvement/research to improve patient outcomes, contributes to the expansion of nursing knowledge, and advances the profession of nursing.
- Participate in the evaluation of current practices at the individual and systems levels identifying gaps in practice and areas for future research to enhance the standard of care and nursing practice.
Education Domain
The NP serves as a clinical resource, educator, and mentor, supporting the interprofessional team, advanced practice learners, and community partners through education, preceptorship, and collaborative consultation.
- Acts as a resource, coach and mentor to the interprofessional team; particularly related to their area(s) of expertise.
- Acts as a preceptor and resource to advanced practice nursing students and may be cross appointed at recognized academic partner institutions.
- Identifies learning needs and uses adult education concepts in planning, designing and implementing educational strategies.
- Provides consultation to community partners to support collaborative practice.
BASIC QUALIFICATIONS:
- Current registration with the College of Nurses of Ontario as Registered Nurses in the Extended Class with one of the following specialty classes: Adult, Pediatric, Neonatal, or Primary Health Care.
- Master of Science (Nursing) or equivalent education
- Minimum 3 years of clinical nursing experience in related area.
- Previous work experience as a Nurse Practitioner (preferred)
- Membership with a professional organization such as CNA, RNAO or NPAO.
- Each NP maintains and continually improves his/her competence by participating in the College of Nurses of Ontario’s Quality Assurance requirements.
- CNA Specialty in area of primary focus (an asset).
- Evidence of exceptional clinical decision-making and critical thinking skills
- Strong knowledge of Nurse Practitioner competencies
- Demonstrated leadership skills within the interprofessional care team
- Excellent interpersonal, communication, presentation and documentation skills
- Demonstrated ability to work both autonomously and collaboratively with all team members
- Evidence of strong mentoring, teaching, team building, change leadership, organizational and prioritization skills, with the ability to work well in stressful situations
- Demonstrated ability to effectively deal with conflict situations and proactively and effectively resolve issues impacting on quality and continuity of care
- Exceptional time management, organizational and prioritization skills
- Demonstrated ability to problem solve and make evidence-based decisions
- Demonstrated ability to interpret/participate in research
- Demonstrated knowledge of computer database, data analysis and presentation packages required

PHYSICAL REQUIREMENTS:
The incumbent will be required to meet the physical demands described for that unit of services.
