Primary Care Networks provide a single integrated system of primary care services to Islanders that are focused on improving health and enhancing access that is community-based and equitable. Network services include primary medical care, diabetes education, chronic disease prevention and management, as well as targeted screening programs. In addition, the following services will initially be available in the Network but managed centrally: primary mental health, nutritional counselling, and public health programs.
The purpose of this position is to maintain, improve, restore and optimize the health status of clients within a practice setting through direct client care for individuals, families, and communities. The Primary Health Care Nurse Practitioner functions autonomously within a collaborative practice setting while providing advanced holistic care of acute and chronic illness to clients across the lifespan.
The Primary Health Care Nurse Practitioner manages a nursing caseload as an integral member of a Primary Health Care Network Team. The Network Team is composed of a variety of health care providers and team composition may vary dependent on identified community needs. The Practitioner works collaboratively with all health care providers. Direct client services provided by the Nurse Practitioner include client assessment, diagnosis and treatment of minor illness, health promotion and chronic disease prevention and management.
The position will support the Primary Care Access Clinic and the East Prince Primary Care Network. The position will report clinically to the Network Clinical Lead and administratively to the Network Manager.
Duties:
- perform comprehensive, focused health assessments including a health history and physical examination using a holistic framework and multiple assessment techniques relevant to a client’s stage of development, culture and health problems;
- analyse client symptoms and assessment data by applying the principles of pathophysiology, pathogenesis, epidemiology, demographics and relevant research;
- determine the need for, order, and interpret the results of relevant screening & diagnostic tests;
- discuss with and involve clients in decision making regarding their health, and in taking responsibility for their health;
- analyse assessment data by systematically compare and contrast clinical, behavioural and diagnostic data to determine differential and working diagnosis;
- determine, communicate and document health findings as well as medical and nursing diagnoses of some diseases, disorders or health conditions;
- discuss with and involve clients in decision making regarding their health, and in taking responsibility for their health;
- provide autonomous coverage as initial point of entry for clients seeking primary health care within the practice setting;
- monitor, adjust, and document the outcomes of drug therapy, interventions and treatment plans;
- provide direct guidance and counseling on health promotion and illness prevention strategies in accordance with client health status and needs, participate in the overall evaluation of the practice setting’s programs and services;
- collaborate with interdisciplinary teams and community members in assessing health promotion and illness prevention needs and provide input as requested for health promotion and illness prevention programs;
- supervise nursing students and participate in the education of other students placed within the practice setting;
- synthesize the information from individuals, client populations and communities to identify broader implications for health within a community;
- ensure professional standards and clinical competence are reflected in all educational content taught by Primary Health Care Nurse Practitioners, and are demonstrated by nursing staff in the work unit;
- home visits or assessments if required;
- maintains a culture of safety within the organization and will be aware of their role in supporting client safety within the organization; and
- other related duties as required.
Qualifications
Minimum Qualifications:
- Must have a Master’s Degree in Nursing as a Nurse Practitioner or graduation from or completed the requirements to graduate from an approved
and recognized Nurse Practitioner program.
- Must have 2-5 years previous community practice or community based Primary Health Care Nurse Practitioner experience.
- Active or eligible for licensure as a Nurse Practitioner with the College of Registered Nurses of Prince Edward Island.
- Maintain CPR certification.
- Previous community practice or community based Primary Health Care Nurse Practitioner experience preferred.
- Extensive nursing experience in a combination of hospital and community settings.
- Previous experience in leadership roles.
- Proficiency and experience working in a computerized environment.
- Thorough knowledge and proficiency in medical and nursing physical assessment and treatment methods.
- Critical thinking proficiency to assess, analyze, diagnose, select treatment, evaluate, and refer patients with common acute or chronic health
conditions, or for health promotion / illness prevention.
- Knowledge of practice setting programs and the community it serves, as well as current grasp of evidence-based primary health care approaches
and epidemiology issues pertinent to the catchment area and available resources.
Other Qualifications:
- Must possess a valid driver’s licence and have access to a reliable vehicle.
- Additional relevant education and experience will be considered an asset.
