Preference will be given to qualified employees within the Full Time Support Staff bargaining unit, Appendix D (6 consecutive months in Appendix D status) and RPT employees who have passed probation at Mohawk College.

The Health & Wellness Systems and Programs Coordinator is designed to strengthen student mental health and wellness by improving how services are delivered, accessed, and experienced. The role sits at the intersection of clinical services, student experience, and operations, acting as a connector across teams. The incumbent will enhance care systems, expand peer and preventative supports, and use data-driven insights to improve service effectiveness. A key goal of the role is to leave behind sustainable infrastructure, processes, and programs that continue to benefit students beyond the term of the contract.
What you’ll be doing:
Service Flow, Alignment & Effectiveness

Participate in case consultation meetings to identify trends and systemic challenges in student needs.

Gather input from the Health & Wellness Centre team members to inform case discussions and service improvements.

Monitor service utilization patterns, including wait times and referral appropriateness.

Identify opportunities to improve triage and care pathways.
Student Engagement & Initiatives
-
Design, implement, and evaluate mental health and wellness initiatives, including peer-led supports.
-
Assist with recruiting, training, and supporting student peer leaders.

Identify and recommend programming aligned with high-demand student needs (e.g., managing stressors, transitioning to post-secondary, belonging).

Contribute to the development of preventative and early-intervention approaches.

Lead student engagement activities such as focus groups, feedback sessions, and needs assessments to inform service planning and initiatives.

Use student insights to inform the development of responsive, accessible programming, and support service improvements and policy recommendations.
Systems Coordination & Service Optimization

Support the improvement of intake, triage, and scheduling workflows to enhance student access and reduce barriers to care.

Review, standardize, and enhance documentation templates, including those used for clinical note-taking in the Electronic Records Management (ERM) system.

Support development of care navigation tools (decision trees, referral maps).

Optimize TELUS Collaborative Health Record (CHR), the ERM system, including:

Intake forms and data fields

Appointment categorization and accuracy

Staff training on best practices
-
Data extraction and reporting
-
Participation in the development of analytics dashboards within the ERM

Explore and support implementation of tools (e.g., AI-assisted note-taking) to enhance workflow effectiveness.
Data, Evaluation & Quality Improvement

Extract, analyze, and interpret service data to inform decision-making and service planning.

Develop and maintain simple dashboards (e.g., wait times, utilization trends, peak periods) to support monitoring, reporting and service planning.

Support and contribute to quality improvement (QI) initiatives to enhance service delivery and outcomes.

Translate data insights into actionable recommendations for service enhancements and process improvements.

Develop and document standard operating procedures (SOPs) to support consistent, efficient service delivery.
Other duties as assigned.
What you’ll bring to the role:

3-year diploma or degree in Public Health, Health Sciences, Nursing, Health Studies, Psychology, Social Work, Health Administration, Health Systems, Health Informatics, Community Health, Human Services, or a related field.

Minimum 3 years of related experience.

Demonstrated commitment and understanding of human rights, equity, diversity, inclusion, and accessibility.

The ability to communicate and work effectively with diverse students, employees, and communities.
