Job Summary
We are seeking a dedicated and experienced Nurse Practitioner to join our healthcare team. The ideal candidate will provide comprehensive patient care across various settings, including primary care, senior care, pediatrics, and acute care environments.
Location: St. Thomas, Ontario (Hybrid: In-Person & Virtual)
Position Type: Independent Contractor / Part-Time (with temporary increased hours during maternity leave coverage)
We are seeking a licensed Nurse Practitioner (NP) to join an established independent primary care practice serving patients across Ontario via virtual care, with a strong commitment to in-person care in a rural community.
This role is ideal for an NP who values continuity of care, autonomy in practice, and a hybrid model that blends virtual and in-person primary care.
Responsibilities
- Conduct thorough patient assessments and develop individualized treatment plans.
- Provide primary and specialized healthcare services across multiple settings such as hospitals, nursing homes, and outpatient clinics.
- Administer medications and perform procedures following established protocols.
- Manage acute and chronic conditions
- Document clinical findings accurately using electronic health records systems JaneApp
- Educate patients and families on health management strategies and preventive care.
- Assist with case management to optimize patient outcomes and resource utilization.
- Maintain compliance with healthcare regulations and standards of practice.
- Provide comprehensive primary care services to a diverse patient population
- Conduct in-person clinical care in St. Thomas at least one weekday per week. You can choose which day of the week this is.
- Deliver virtual primary care visits to patients located throughout Ontario
- Manage acute and chronic conditions, preventative care, and health promotion
- Order and interpret diagnostic tests, prescribe medications, and make referrals as appropriate
- Maintain accurate, timely clinical documentation
- Collaborate with the primary NP to ensure seamless continuity of care
- Provide coverage during the primary NP’s maternity leave, with increased availability during that period
Experience
- Proven experience working with children, seniors, or in hospital settings is highly desirable.
- Experience in acute care, pain management, dementia care, or home care environments is advantageous.
- Familiarity with medical office procedures, medical terminology, medication administration, and laboratory testing
- Proficiency with electronic health record systems
- Prior experience in pediatrics or senior care enhances the ability to deliver specialized patient-centered services.

Qualifications
- Registered Nurse Practitioner licensed and in good standing with the College of Nurses of Ontario (CNO)
- Eligible to practice independently in Ontario
- Experience in primary care (family practice experience strongly preferred)
- Comfortable with virtual care platforms and remote patient management
- Strong clinical judgment, communication, and organizational skills
- Ability and willingness to work on-site in St. Thomas at least one weekday per week
- Willingness to provide temporary maternity leave coverage
- Need PLP coverage whether through CNPS or NPAO or RNAO
Practice Model
- Independent contractor role, fee-for-service
