Position Summary
The Community Outreach Worker provides structured, community-based outreach, education, and support services related to substance use and wellness across the Nlaka’pamux Nation. This role focuses on planned, relationship-based outreach through the facilitation of pop-up education sessions and community engagement in health clinics and gathering spaces.
Unlike crisis-response roles, this position emphasizes proactive engagement, health education, and connection to services, creating safe and supportive spaces where individuals and families can access information, build skills, and strengthen connections to care.
Working under the direction of the Community Wellness Manager, the Community Outreach Worker supports individuals, families, and communities by providing education, facilitating group discussions, and connecting people to health, wellness, and cultural supports.
Key Responsibilities
Community Outreach & Engagement
- Provide outreach across the Nlaka’pamux Nation communities to support individuals experiencing substance use challenges
- Conduct planned, non-crisis outreach activities focused on education, engagement, and connection to services
- Maintain a consistent, respectful, and culturally safe presence in community settings
- Build strong relationships with individuals, families, and community members
- Identify opportunities to engage individuals who may not be accessing services
Pop-Up Education Sessions & Group Facilitation
- Plan, coordinate, and deliver pop-up wellness and education sessions at community health clinics and gathering spaces
- Facilitate structured group sessions on substance use, wellness, harm reduction, and available supports
- Create safe, inclusive environments that encourage respectful discussion and peer connection
- Establish clear group guidelines to support safe and productive conversations
- Provide sessions tailored to families supporting individuals experiencing substance use
Harm Reduction & Wellness Education
- Provide education on harm reduction, safer use practices, and overall wellness
- Promote overdose prevention awareness and knowledge of available community resources
- Support informed choice, safety, and dignity in all interactions
- Integrate culturally grounded approaches into all outreach and education
Navigation, Referral & Support
- Support individuals in accessing health care, treatment, cultural services, and community supports
- Provide warm referrals and follow-up where appropriate
- Work collaboratively with Mental Health, Nursing, and Wellness teams
- Assist individuals and families in navigating services and systems
Collaboration & Team Integration
- Work closely with interdisciplinary teams to support coordinated, wraparound care
- Participate in team meetings, case discussions, and service planning
- Share relevant information while maintaining confidentiality and professional boundaries
Documentation, Reporting & Quality
- Maintain accurate, timely records of outreach activities and engagement
- Track participation, outreach efforts, and program outcomes
- Support program reporting and evaluation requirements
- Ensure documentation aligns with NHSS Health policies and funding expectations
Culturally Safe & Community-Driven Practice
- Deliver services grounded in cultural safety, humility, and trauma-informed practice
- Respect Nlaka’pamux knowledge, traditions, and community protocols
- Build and maintain strong relationships with Elders, families, and community partners
- Support community-led approaches to wellness and healing
Qualifications
Required
- Diploma in Addictions, Human Services, Social Services, Community Support, or a related field (or equivalent combination of education, training, and experience)
- Experience in community outreach, substance use, harm reduction, or social services
- Experience facilitating group sessions, workshops, or community-based education
- Strong communication, facilitation, and relationship-building skills
- Ability to work independently in community-based environments
- Ability to maintain professional boundaries and respond appropriately in complex situations
- Valid Class 5 Driver’s Licence and ability to travel across the Nlaka’pamux Nation
Assets
- Experience working with Indigenous communities
- Knowledge of Nlaka’pamux culture, values, and traditions
- Training in harm reduction, trauma-informed practice, or substance use supports
- Mental Health First Aid, ASIST, or First Aid (or willingness to obtain)
- Experience supporting families impacted by substance use
Working Conditions
- Community-based, outreach-focused role across the Nlaka’pamux Nation
- Regular travel between communities
- Facilitation of group sessions and public-facing activities
- Occasional evenings and weekends based on community needs
- Work may involve supporting individuals experiencing complex needs
