Summary
The Case Manager /Facilitator is pivotal in empowering individuals with medical conditions and barriers to employment to navigate employment challenges by conducting assessments and creating tailored action plans that steer them toward meaningful career paths. They provide holistic support through case management and proactive follow-ups to ensure long-term success. They strategically build and foster understanding by creating impactful connections to build a more inclusive and supportive employment landscape.

Intake and Assessment
- Conduct individual assessment interviews to determine client selection for the Restart Your Career program
- Assist in the identification of an individual’s employment barriers, employability and learning needs
- Request additional mental health/medical/physical documentation from attending professionals to support the individual’s participation in the program
- Provide referrals/suggestions for alternative programs and services
- Assist the client in acceindividuals'ssing necessary and appropriate interventions and social supports
- Assist in the final selection of individuals for the current intake
- Ensure accepted individuals application information is entered into the Foundational Learning (FL) Student Management System for funding approval
Case Management & Employment Counselling
- Ensure that all attendance and progress policies are adhered to and recorded appropriately
- Provide reminders to individuals regarding book fees and payments to the Admin
- Maintain client case management activities, providing referrals and recommendations to external services as needed
- Maintain client database (Apricot) and files, ensuring individual, program and statistical information is up to date,
- Ensure that participants' withdrawals are entered into the FL Student Management System immediately
- Solicit and provide feedback regarding participants' issues and concerns as needed and requested, and complete monthly progress reviews
- Promote the use of career development and vocational rehabilitation theory and practice to assist participants with realistic employment goals
Facilitation and Training
- Promote and deliver in-person group workshops to prepare clients to explore, secure and maintain employment.
- Research, develop and enhance program curriculum and prepare instructional materials to meet the needs of participants.
- Assist with the development of Restart Your Career program calendar
- Assist clients with technology needs as required.
- Secure, schedule and support sessions offered by external instructors and guests.
Employer Placement
- Work with the job development team on all activities related to marketing participants, including work practicums.
Community Involvement
- Represent the Agency and promote available services and programs to the communities.
- Participate in networking, presentations, conferences, and committees.
- Attend job fairs, labour market and other events.
Section 4: Summary of Qualifications
Educational Background:
- Diploma/Certificate in related field
- Required to have a valid class 5 driver’s license and access to a vehicle or reliable public transportation.
- Clear criminal record check, including vulnerable sector check
Technical Skills:
- Proficiency in the use of Microsoft Office 365.
- Knowledge and understanding of database programs are assets.
- Proficiency in virtual meeting platforms (Skype, Zoom, Teams, Google Meet, etc.)
Job Expertise/Experience:
- Previous experience in intake and assessment, case management, adult workshop facilitation, and employment counselling is considered an asset.
- Previous experience working with persons with disabilities or other at-risk populations preferred.
