Home Base Housing is in the business of ending homelessness. We do this by caring and providing individual wrap-around services delivered by case managers to the vulnerably housed and homeless. With over 22 properties under our management, we provide emergency, transitional and supportive housing, for youth, families, and single adults, the One Roof Youth hub, a housing help centre, street outreach, housing loss prevention services, and advocate for affordable housing for all.
About the job
The Case Manager- builds a relationship with the client in order to develop a support plan which includes identifying existing barriers to self-sufficiency, setting goals and developing a realistic action plan.
The Case Manager position follows an alternating weekly schedule of days and afternoons.
The Case Manager assists the client to obtain and maintain the external resources needed by providing information, making appropriate referrals, and acting as a liaison between client and various providers. The Case Manager strives to promote a positive, supportive, and respectful relationship with the client at all times.
Duties and Responsibilities:
Application/ Intake Assessment related:
works with the client to identify the initial support needs and the presenting strengths and barriers to addressing the needs
identifies other service providers and natural supports (e.g. family) and obtains consents to share information
obtains consent to share information and liaise with other community agencies to develop and implement the initial support plan in a coordinated manner
prepares the client support file and completes in a timely manner all documentation related to application/intake assessment
advocates for and refers individuals who are ineligible for the program or service helping them gain access to other programs and services at Home Base Housing and/or resources in the community
Client Support and Liaison related:
providing supports to clients in their homes -clients are not expected to, nor are they required to, go to an office to receive case management or any other supports;
developing a housing stability plan, a focus on housing stability in the early stages of the program, which focuses on meeting basic needs, the supports required to maintain housing, safety, and the impact of the client’s relationships on housing tenure;
assisting the client to secure financial assistance to meet tenancy obligations, such as paying rent and needed household items as required including assistance with completion of application for Homelessness Prevention Fund as appropriate;
completing a personalized Crisis Plan with the client within the first four weeks of living in the housing unit;
assessing risks presented by the client and develop appropriate plans to help ensure continuation of services and mitigate risk;
creating wellness plans in cooperation with mental health and/or allied professionals and assisting clients in incorporating the wellness plan activities;
completing with the client, the SPDAT at the intake assessment and at predetermined intervals of service delivery (at move in, 30 days, 90 days, 180 days, 270 days, 365 days, and changes in life circumstances);
using other case planning tools that may be deemed necessary and appropriate by the Case Manager and Program Manager during case planning such as the Outcome Star, Recovery Star, Readiness Ruler, Decision Scales, and so forth which are subject to the interest of the client,
writing an Individualized Service (Recovery) Plan identifying the supports required and agreed to by the client;
convening client case conferences with other agencies, on an as needed basis, for each client on their caseload relative to the client’s individualized plans and progress.
Community Development related:
promotes the work of Home Base Housing through community education
identifies gaps in services and advocates in the broader community to encourage change consistent with the philosophy of Home Base Housing
participates in community committee work which advances the goals of Home Base Housing
researches and updates information on community resources
Other Duties and Responsibilities:
notifies Program Manager and office if absent from work
participates as a full member of the team, sharing knowledge and being willing to be influenced by the opinion of others
substitutes for other staff as assigned
participates in team meetings and team activities
actively seeks out professional development activities relevant to the position
advise the Program manager/Supervisor of issues that impact either positively or negatively on their work with suggestions for addressing these
Skills, Knowledge, and Education Required
demonstrated knowledge of homelessness, mental health/ addiction issues, and resources available
solid judgment and problem-solving skills
ability to work independently and as part of a team
ability to work collaboratively in community settings
good oral and written skills
computer-competency in areas relevant to job expected
knowledge of housing, health care and community resources in Kingston preferred
Qualifications
University degree and/or Community College Diploma in Social Services or related field
3 years experience working with the population is preferred
Valid Ontario Driver’s License and access to a car
CPR/First Aid Certification required prior to employment
