Overview
Bridges to New Life Society is seeking a Social Worker (student position) to support individuals transitioning from incarceration into the community. This role focuses on providing practical reintegration support, helping men navigate barriers such as poverty, housing instability, lack of identification, and limited access to employment and community resources.
The Successful candidate will work within a supportive, interdisciplinary team to assist clients in accessing services, building stability, and developing pathways toward successful community reintegration.
Key Responsibilities
- Conduct client intakes and assist in developing individualized reintegration plans (housing, employment, income support, ID).
- Connect clients with community resources, agencies, and support services.
- Assist clients in navigating systems such as social assistance, housing, healthcare, and employment services.
- Maintain accurate and confidential client records, including case notes and progress tracking.
- Support program activities such as life skills development, application, and referrals.
- Participate in interdisciplinary team meetings and contribute to client care planning.
- Contribute to a safe, inclusive and supportive environment for clients from diverse backgrounds.
