Enhanced Care Facilitator – Contract, 6 months (25 hours/week)
Community Care City of Kawartha Lakes (CCCKL) is currently seeking a Part-Time (Contract) Enhanced Care Facilitator to join our dedicated Community Support Services team. Based at Ross Memorial Hospital, this position plays a vital role in supporting the Home First Hospital to Home initiative by providing assessment, coordination, and outreach services that promote independence for frail seniors and individuals living with complex and chronic care needs. The successful candidate will work collaboratively with hospital staff, community partners, clients, and families to ensure safe and effective transitions from hospital to home. KEY ACCOUNTABILITIES:
- Receives and processes referrals for services from Ross Memorial Hospital and community care partners
- Uses professional knowledge and experience to contribute to interdisciplinary team decision-making
- Participates in meetings and communicates effectively with hospital staff, clients, families, and community partners
- Conducts assessments to determine eligibility for Enhanced Services and accessibility supports to facilitate timely hospital discharge
- Arranges and coordinates client services within program guidelines and funding parameters
- Provides consultation, problem-solving, and follow-up support as needs arise
- Maintains regular contact with clients to assess emerging or ongoing support needs
- Schedules and conducts home visits as required
- Coordinates referrals and transitions to ensure continuity of care from hospital to home
- Works collaboratively with program staff to coordinate transportation, meals on wheels, friendly visiting, adult day programs, respite services, and other community supports
- Maintains knowledge of community services and resources to support client needs
- Prepares and maintains client documentation, intake records, referrals, and case notes in electronic database systems
- Completes scheduling, statistical, billing, and narrative reporting requirements as required
- Develops and maintains effective relationships with hospital staff, community agencies, housing providers, and service organizations
- Participates in educational sessions, committees, meetings, and quality improvement initiatives
- Maintains strict confidentiality and ensures compliance with privacy legislation, organizational policies, and infection prevention and control standards
- Promotes a client-centered approach and contributes positively to team effectiveness and organizational culture
- Works in compliance with Occupational Health and Safety legislation and organizational safety policies
KEY COMPETENCIES REQUIRED FOR THIS POSITION
- Post-secondary education (university) in Health Services, Nursing, or an allied health discipline preferred
- Registration with a regulated College required
- Minimum three years’ experience coordinating support services, or an equivalent combination of education and experience
- Experience working within the community support services sector
- Experience conducting client assessments and developing care plans for frail seniors and individuals with special needs
- Knowledge of community service providers and health care resources
- Strong leadership, coaching, mentoring, conflict resolution, and problem-solving skills
- Strong documentation, organizational, planning, and prioritization skills
- Knowledge of privacy legislation and consent requirements
- Excellent interpersonal, verbal, and written communication skills
- Well-developed computer and keyboarding skills, including experience with electronic documentation systems
- Demonstrated commitment to continuous learning and professional development
- Valid Ontario driver’s license and access to a reliable vehicle
- Satisfactory Police Record Check with Vulnerable Sector Screening required
COMPENSATION:
The wage range for this exciting opportunity is $26-89-$29.44 per hour.Selected candidates will be appropriately placed on the organizations wage grid based on skill, knowledge and experience.*
