Responsibilities:
- Acts as the initial contact for all incoming calls from patients, families, community agencies and physicians for the CBC Clinic
- Review incoming referrals, complete assessments (physical, psychological, social, cultural, and spiritual needs) both on the phone and in person and provide symptom management, teaching, and emotional support for patients and families within the program. Conducts a health assessment of patient and determines clinical needs in collaboration with the CBC Clinic
- Documents, tracks and prioritizes care requests to ensure all patients and families receive a timely response
- Required to collect and report volumes, diagnostic wait times, and symptom management data to JBH, CIHI, NACRS, and Cancer Care Ontario and be responsible for initiatives to meet and exceed benchmarked metrics
- Completes follow-up calls to patients and/or families to ensure care needs have been met
- Liaises with CCAC, community agencies, clinics, pharmacies and family physicians
- Prepares reports and participates in all team meetings and rounds
- Patient and family teaching and surgery planning
- Facilitating breast path-rad weekly rounds
- Maintains and updates all patients medical records and files including patient and family risk indicators
- Uses critical thinking skills to escalate more urgent patient care needs
- Supports the team with respect to research activities and learner placements
Qualifications:
- Current Certificate of Competence in the Province of Ontario as a Registered Nurse
- Minimum 5 years acute care clinical experience. Surgery or Oncology experience preferred.
- Experience tracking program metrics and implementing successful strategies to meet exceed benchmarks
- Demonstrated ability to work independently or as part of an interprofessional team
- Excellent interpersonal and conflict resolution skills
- Knowledge of support services and resources in the community
- Excellent written and verbal communication skills
- Knowledge of Word, Excel and Meditech
- Demonstrated reliable attendance, punctuality, availability and performance required
