LPN Care Manager, Program Manager Key Responsibilities
- Develop and oversee individualized care plans that address residents’ unique needs.

LPN Care Manager, Program Manager Qualifications and Experience

- Minimum 3 years of practical nursing experience, preferably in senior care, dementia, acute care, or pediatrics.
- At least 2 years of nursing leadership or supervisory experience preferred.
