About Dorothy Ley Hospice
Dorothy Ley Hospice (DLH) has been caring for community members in Etobicoke, West Toronto and Mississauga for more than 35 years. As one of Ontario’s oldest and most established hospices, we’re helping to lead the future of hospice palliative care. We offer community and hospice residential palliative care, as well as grief and bereavement support. Our compassionate team of palliative care physicians, nurses, staff, and volunteers provides support and care to over 2,000 individuals annually.
The Opportunity
Reporting to the Director, Community Programs & Quality, the Psychosocial Spiritual Care Counsellor (PSC Counsellor) is accountable for providing psychosocial and spiritual care support/counselling to people living with serious illness, caregivers, and those who are bereaved, in the DLH residence and in the community. The incumbent will assess client needs and co-create a psychosocial spiritual support plan with the individual. This includes support for spiritual care needs such as existential questions, meaning and purpose, belief or religious crises, legacy work and culturally based needs that pertain to dying and death.
Joining with a dedicated and caring bereavement team, the PSC Counsellor will also provide service that is psychosocial focused by utilizing the spiritual health principles (e.g. dignity, meaning making, beliefs, tolerance, holism, etc.) to improve wellness. The PSC Counsellor also provides general grief and bereavement support and counselling to individuals experiencing anticipatory or post death grief.
Key Responsibilities
- Assess psychosocial and spiritual needs, develop and maintain care plans and provide counselling using a person-centered, trauma informed, and spiritually and culturally appropriate approach to care
- Deliver individual psychosocial and spiritual support and counselling to individuals living with life-limiting illness, family caregivers, and people who are bereaved
- Provide appropriate opportunities for spiritual, religious or cultural rituals
- Facilitate access to spiritual support through community faith and/or cultural groups
- Lead the delivery of remembrance events and facilitating support groups
- Provide one-on-one bereavement support/counselling and anticipatory grief support
- Identify and respond appropriately to risk (e.g., suicide ideation, complicated grief, trauma responses) in collaboration with the clinical team
- In collaboration with the Volunteer Management department, offer support and mentorship to spiritual care volunteers
- Deliver spiritual care and grief education presentations and workshops
- Contribute to the development of evidence informed psychosocial and spiritual programming and curriculum development
Qualifications
- Certification with Canadian Association for Spiritual Care (CASC) or equivalent work experience as recognized by the Hospice.
- Minimum two (2) years experience delivering spiritual care
- Experience supporting end-of-life and experience with bereavement counselling
- Experience supporting individuals from diverse cultural, spiritual, and religious worldviews
- Knowledge of and connection to a range of faith groups and faith leaders
- Ability to work flexible hours, including some evenings and weekends
- Valid driver’s license and access to a reliable vehicle, with insurance covering driving for business purposes and Vulnerable Sector Police check required
