Stoney Health Services (SHS) provides community-based health and wellness services to the Stoney Nakoda Nations of Bearspaw, Chiniki and Goodstoney. SHS works collaboratively with individuals, families, community partners and health care providers to support culturally responsive, client-centred care, health promotion, prevention and access to services while respecting Stoney Nakoda culture, language, traditions and community priorities.
Job Summary:
The Stoney Health Services (SHS) Harm Reduction Licensed Practical Nurse (LPN) works collaboratively with the SHS multidisciplinary team to provide compassionate, confidential and non-judgmental support to clients, families and the significant others of those who use substances. The focus is to decrease the adverse health, social and economic consequences of substance use without requiring abstinence; to empower clients to define their own needs with regard to their care; provide information and encourage public understanding of those who use substances.
Scope:
Working in accordance with the College of Licensed Practical Nurses of Alberta (CLPNA) Standards of Practice, and within the context of a client-centered care model, a harm reduction framework, and the vision/values of Stoney Health Services, the LPN works with clients who may present with a variety of acute and chronic illnesses/diseases, including communicable diseases, addictions and general health issues. The LPN collaborates with interdisciplinary team members, educational resources and other health care professionals/providers to achieve excellence in client-centered care and education. Specific duties and responsibilities may include:
Skills and Abilities:
· Providing education and counselling using a harm reduction approach to clients regarding safer substance use; and in such matters as sexual health, including risk and prevention of sexually transmitted and bloodborne infections (STBBI’s);
· Providing direct nursing care to clients at SHS locations, clients homes and in the community as required
· Distributing safe injection and substance use supplies;
· Supporting clients on medication replacement therapy including initiation, ongoing support and referrals for health maintenance from appropriate health care providers;
· Carrying out confidential testing and treatment of STBBIs including appropriate reporting as required by legislation.
· Working collaboratively with health care professionals, clients and community organizations in the identification and resolution of health issues and integration of care, and makes referrals to other service providers;
· Providing group and community-based education, information and presentations, as required;
· Supporting the development of Harm Reduction projects and events;
· Collaborating in the activities of the Sexual and Reproductive Health Program; and other SHS programs as required
· Maintaining accurate, detailed, timely and confidential records of nursing actions
· Carrying out administrative and clerical functions related to his/her role;
· Maintaining an effective data collection and reporting system as well as providing regular reports and recommendations to SHS management, as requested;
· Carrying out other associated duties as may be required and/or directed (i.e. policy reviews, grant writing,
· leading groups etc.)
· Developing and maintaining a network (internal and external) of collaborative working relationships with individuals, communities as well as territorial, provincial or federal organisations
· Participating, as a member of the Stoney Health Services team, in:
· Quality Improvement, Risk Management and Accreditation activities;
· Community outreach, educational and other health promotion /injury prevention activities etc.;
· Team / staff meetings, training and development opportunities;
· Operational and strategic planning sessions
· Acting as an ambassador for SHS, for client and family-centered care and as advocate for client autonomy;
· Complying with all relevant SHS and Stoney Tribal Administration policies, procedures and regulations;
· Performing other associated duties as required and/or directed.
Other areas of demonstrated knowledge and competence are as follows:
§ Registration in good standing with the College of Licensed Practical Nurses of Alberta;
§ A thorough knowledge of current Harm Reduction practices;
§ Experience with or interest in Indigenous culture;
§ Ability to successfully manage multiple projects and programs and priorities;
§ Strong communication (oral and written), relational, coordination and organizational skills;
§ An understanding of cultural diversity issue, benefits and strategies to enhance relations;
§ A proven ability to work well with individuals from varying backgrounds;
§ Computer skills relevant to the position;
§ Holds a current Class 5 Alberta Drivers License and a Clean Driving Record (Conditions of Employment);
Educational Requirements:
· A graduate in good standing of a recognized Licensed Practical Nurse program;
· Current Cardio-Pulmonary Resuscitation (CPR) Certification BLS - Level C);
· Immunization Certification (CLPNA), preferred;
· Phlebotomy experience; preferred
· Maintains currency through ongoing professional development, professional affiliations, participation on pertinent committees, professional conferences, e-learning, journal reviews etc.

· Must have a valid Driver’s license and regular use of a vehicle
Work Environment:
· Fast paced First Nation office environment
· Travel may be required
Hours of Work
· 35 hours per week, Monday to Friday, 9:00 a.m. to 4:30 p.m.
