Family Service Nova Scotia (FSNS) is seeking a Youth Outreach Plus Worker to provide intensive, community-based support for youth experiencing complex challenges across the social determinants of health. Working alongside youth, families, schools, community organizations, and service providers, this position delivers goal-focused case management, crisis response, and advocacy that empowers young people to build stability, strengthen connections, and achieve meaningful outcomes. The successful candidate brings strong relationship-building skills, sound professional judgment, and a collaborative approach, balancing direct youth engagement with reflective practice, documentation, and coordinated service planning to ensure responsive, high-quality support.
Program Overview
The Youth Outreach Plus (YOP+) program provides Tier 3, time-limited case management services for youth aged 12 to 26 experiencing multiple and complex needs. Using a strengths-based, trauma-informed approach, Youth Outreach Plus works alongside young people to reduce barriers, build resilience, and increase access to the supports and opportunities needed to achieve their goals.
The program offers individualized support through community outreach, family engagement, advocacy, and system navigation. Participants also have access to 24/7 on-call support, delivered through Family Service Nova Scotia's Helplines and daytime Youth Outreach Plus staff, ensuring youth receive timely assistance when they need it most.

This full-time (35-hours) position works in person in Antigonish. The annual compensation for this position is $70,000.
What You'll Do
1. Youth Outreach and Case Management
- Complete comprehensive assessments, needs assessments, and individualized case plans.
- Build trusting relationships with youth through community, home, office, and virtual visits.
- Provide intensive, goal-focused case management and ongoing support.
- Deliver supportive counselling and strengths-based interventions.
- Engage families and caregivers as appropriate to strengthen natural supports.
- Connect youth with community resources through referrals, advocacy, and system navigation.
- Help youth access basic needs, including housing, food, transportation, education, employment, and healthcare.
- Provide transportation and accompaniment to appointments and community services when required.
- Monitor progress and adapt support plans based on changing needs and goals.
- Gather participant feedback and outcome data to support continuous program improvement.
- Manage program funding responsibly to address youth-specific needs.
2. Collaboration and Clinical Consultation
- Work closely with Youth Outreach Workers, schools, healthcare providers, community organizations, and other service partners.
- Facilitate regular case consultations and referral discussions with Youth Outreach teams.
- Provide consultation and support to Youth Outreach Workers as appropriate.
- Participate in multidisciplinary planning to ensure coordinated service delivery.
3. Reflective Practice and Professional Development
- Participate in individual reflective supervision and group supervision.
- Attend monthly clinical supervision and engage in ongoing case consultation.
- Apply reflective practice to strengthen decision-making, service quality, and professional growth.
- Participate in team meetings and ongoing learning opportunities.
4. Administrative Responsibilities
- Maintain accurate, timely, and confidential case documentation.
- Complete monthly statistics, outcome reporting, and required program documentation.
- Record Helpline interactions and maintain program tracking systems.
- Monitor program budgets and complete expense reporting as required.
- Manage scheduling, appointments, and case coordination activities.
5. Organizational Support
- Contribute to a positive, collaborative team culture.
- Participate in community committees, Youth Outreach Program meetings, and partnership initiatives.
- Support organizational priorities and special projects as needed.
- Participate in internal professional development and knowledge-sharing opportunities.
What You Must Have
- Bachelor of Social Work (BSW).
- Relevant experience working with youth experiencing complex social, emotional, behavioural, or mental health challenges.
- Eligibility for registration with the appropriate professional regulatory body.

- Availability to work flexible hours, including evenings and participation in a 24/7 on-call rotation.
