Summary
We are seeking a dedicated Nurse Practitioner to join our team at Owen Sound Gardens Retirement Residence. In this role, you will play a vital part in delivering high-quality healthcare to our residents, ensuring their health and well-being while fostering a supportive environment.
Responsibilities
- Conduct comprehensive assessments of residents' health status and develop individualized care plans.
- Provide direct patient care, including diagnosis and treatment of acute and chronic conditions.
- Collaborate with other healthcare professionals to ensure coordinated care for residents.
- Educate residents and their families about health management and wellness strategies.
- Monitor residents' progress and adjust care plans as needed.
- Participate in case management and quality improvement initiatives.
- Involved in Meet and Greets with the residents quartly.
Requirements
- Registered Nurse (RN) designation with a Nurse Practitioner (NP) certification.
- Strong experience in geriatrics or long-term care settings preferred.
- Excellent critical thinking and problem-solving skills.
- Ability to work collaboratively within a multidisciplinary team.
