Job Summary
We are seeking a compassionate and dedicated Case Worker specializing in Mental Health and Addictions to join our team. The ideal candidate will provide support, guidance, and resources to individuals facing mental health challenges and substance use issues. This role involves working closely with clients to develop personalized care plans, facilitate access to services, and promote recovery and well-being. Supervising experience is a valuable asset for this position, ensuring effective oversight of case management activities and team collaboration.
Duties
- Conduct thorough assessments of clients’ mental health and addiction needs
- Develop individualized care plans in collaboration with clients and multidisciplinary teams
- Provide ongoing support, counseling, and crisis intervention as needed
- Coordinate services such as therapy, medical treatment, housing, and employment assistance
- Monitor client progress and adjust care plans accordingly
- Maintain accurate documentation of all client interactions and services provided
- Facilitate group sessions or support groups related to mental health and addiction recovery
- On Call one week on one week off shared with Operations manager.
- Advocate for clients’ needs within community resources and service providers
- Community outreach with clients expected.
Qualifications
- Diploma in Social Services, Psychology, Counseling, or a related field
- Diploma Mental health and addictions
- Proven experience working with individuals facing mental health or addiction issues
- Flexible for on call duties.
- Strong interpersonal and communication skills to build trust with clients and collaborate with teams
- Ability to handle sensitive situations with empathy, professionalism, and confidentiality
- Knowledge of community resources, treatment options, and best practices in mental health and addiction services
- Ability to work independently as well as part of a multidisciplinary team
- Valid license or certification relevant to mental health or social services is a plus
Pay: $21.50 per hour
