Job Summary
We are seeking a dedicated PSW to provide compassionate and comprehensive nursing care in a long-term care setting in Lucknow . The ideal candidate will have experience in nursing home environments and a strong background in patient-centered care, including dementia, senior care, and hospice services. This role involves collaborating with interdisciplinary teams to ensure the well-being and safety of residents while maintaining high standards of clinical practice.
Duties
- Assist with activities of daily living, including hygiene, mobility, and nutrition support
- Document patient care activities thoroughly using electronic health records such as Epic or Cerner systems
- Collaborate with physicians, therapists, and family members to develop and update individualized care plans
- Provide specialized care for residents with dementia, hospice needs, or other chronic conditions
- Educate residents and families about health conditions, medication management, and discharge instructions
- Ensure compliance with healthcare regulations and safety standards within the facility
Qualifications
- Valid PSW diploma from an Ontario School
- Prior experience in nursing homes
- Experience working with diverse populations including seniors, or hospice patients
- Valid VSC within the last 6 months
- Tb test within last 6 months
- MMR
- CPR
