Overview
We are seeking a compassionate and experienced Registered Nurse (RN) to join our team specializing in complex needs and community transitions. This role offers an opportunity to provide high-quality patient care across diverse settings, including home care, assisted living, nursing homes, and pediatric environments. The ideal candidate will possess strong clinical skills, excellent communication abilities, and a commitment to improving patient outcomes through comprehensive case management and personalized care plans. Experience with electronic health records such as Epic and Cerner is preferred, along with a background in hospital or acute care settings.
The RN will work closely with Program Managers, Complex Needs Coordinators, physicians, psychiatrists, occupational therapists, social workers, behaviour consultants, and frontline staff to ensure safe, coordinated, person-centered care.
Responsibilities
- Deliver direct patient care across various settings, including home health, assisted living, nursing facilities, and pediatric environments.
- Administer medications accurately following prescribed protocols and document all interventions in electronic health records.
- Conduct assessments of patients' health status, including vital signs, pain management, and laboratory procedures such as phlebotomy.
- Develop and implement individualized care plans tailored to patients' complex needs, including dementia care and senior care.
- Collaborate with interdisciplinary teams to coordinate patient transitions from hospital to community settings, ensuring seamless continuity of care.
- Educate patients and their families on medication administration, disease management, and safety protocols.
- Maintain compliance with healthcare regulations and documentation standards using systems like Epic or Cerner.
- Monitor patient progress and adjust care strategies accordingly to optimize health outcomes.
- Assist in managing medical office tasks related to patient scheduling, record keeping, and medical terminology documentation.
Requirements
- Valid RN license in the state of practice with current CPR certification.
- Proven experience in hospital or acute care environments; experience in pain management, case management, pediatrics, or senior/dementia care is highly desirable.
- Proficiency with electronic health record systems such as Epic or Cerner.
- Strong knowledge of medical terminology, medication administration protocols, and laboratory procedures including phlebotomy.
- Excellent communication skills for working effectively with patients of all ages—including children—and multidisciplinary teams.
- Ability to manage complex cases involving multiple comorbidities and coordinate community transitions smoothly.
- Experience working in home care settings or medical offices is a plus; familiarity with assisted living or nursing home environments is preferred.

QUALIFICATIONS
Required:
- Active registration with CRNA/CARNA
- Minimum 3 years nursing experience
- Community health, home care, mental health, or disability services experience
- Valid Class 5 Driver’s Licence

Pay: $45.00-$50.00 per hour
