The OakMed Family Health Team (OFHT) is seeking a passionate and skilled Nurse Practitioner (RN-EC) to join our dynamic, patient-centered primary care team. We serve a diverse population in the Oakville area, providing interdisciplinary, compassionate care focused on illness prevention, chronic disease management, harm reduction, and health promotion.

Position Highlights
- Full-time hours: Monday to Friday, 9:00 AM – 5:00 PM
- One evening shift every other week (Wednesday, 5:00 – 8:00 PM, with flexible scheduling)

Key Responsibilities
- Provide acute and episodic care for patients of all ages
- Conduct well-baby visits and well-woman exams
- Deliver general family health care
- Collect clinical data as required for reporting and evaluation
- Contribute to the overall efficiency and smooth operation of the OakMed FHT
- Collaborate effectively with the interdisciplinary team in a fast-paced environment
Qualifications
- Licensed and in good standing to practice as an RN(EC) in Ontario
- Experience in primary care setting preferred
- Excellent clinical, organizational, and problem-solving skills
- Demonstrated ability to prioritize tasks, maintain confidentiality, and adapt to a changing environment
- Strong interpersonal and communication skills; team-oriented with the ability to work independently
- Experience using Electronic Medical Records (EMR); knowledge of TELUS PS Suite is an asset
- Detail-oriented with sound judgment and a proactive attitude
