Summary:
The Case Manager is an integral member of a multidisciplinary team supporting clients with severe and persistent mental illness. This role focuses on client-centered care through assessments, treatment planning, health promotion, medication management (in coordination with the team psychiatrist), and ongoing support aligned with company's standards.
Key Responsibilities:
- Initiates services to clients at the point of intake to ensure that proper assessment and initial service response occurs;
- Develops clinically sound case plans, using appropriate assessment tools, that are specific to the needs of clients and the objectives of the program in question;
- As the Primary Case Manager or as a member of a client care team, ensures implementation and evaluation of the care plans, based on a team approach;
- As the Primary Case Manager, ensures that all administrative functions related to her/his client list are completed;
- Establishes case management links with other resources
- Pursues case management activities independently and in concert with other professionals, agencies and groups;
- Maintains accurate and complete clinical records;
- Provides leadership in ensuring case conferences occur as required;
- Collaborates with other appropriate service systems, organizations and individuals to ensure that services are well co-ordinated and meet a high standard of quality care and/or supervision;
- Identifies problems and possible solutions for resource use, and collaborates for service improvement;
- Assesses functional and resource needs in relation to the needs and goals of clients;
- Delivers all services in a professional manner and according to established guidelines and associated benchmarks;
- Follows all health and safety guidelines established by Joint Health &; Safety Committee;
- Fulfills other responsibilities as required, possibly for clients of other programs.
Qualifications:
Education:
- Post-secondary education is required. This can be a Community College Diploma in Social Services, a University Degree in Social Sciences, or a combined University Degree and Community College Diploma in Human Services.
Experience:
- Appropriate experience in a related field.
- Work experience in a community mental health setting.
- Experience working with Psychosocial Rehabilitation (PSR) models.
- Experience in individual counseling.
- Experience in direct service delivery to adults with serious mental illness.
Knowledge:
- Good understanding of mental illness issues, the mental health system, and concepts related to the effects of serious mental illness.
- Familiarity with relevant legislation (e.g., Mental Health Act, Personal Health Information Protection Act).
- Knowledge of local housing, healthcare, and community resources.
- Knowledge of and sensitivity to gender, race, sexual orientation, age, and multicultural issues.

- Ability to work independently and as part of a team.
- Ability to work collaboratively with community resources.
Other Requirements:
- Regular access to a dependable vehicle.
- A valid Ontario driver's license in good standing.

Pay: From $38.00 per hour
