Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality patient care across various settings, including nursing homes, hospitals, assisted living facilities, and home care environments. This role requires a strong foundation in nursing practices, excellent communication skills, and the ability to work effectively with diverse patient populations, including seniors, children, and those requiring specialized care such as dementia or hospice. The LPN will play a vital role in supporting patient health and safety through direct care, medication administration, and collaboration with interdisciplinary teams.
Duties
- Administer medications and treatments accurately following physician orders and established protocols
- Monitor patient vital signs and assess health status regularly
- Assist with daily living activities and provide compassionate patient care tailored to individual needs
- Document patient information accurately using electronic health records systems such as ShiftCare
- Support patients with dementia, hospice, pediatric, or senior care needs with patience and professionalism
- Collaborate with physicians, nurses, and other healthcare professionals to develop and implement care plans
- Educate patients and their families on health management, medication use, and post-discharge instructions
- Maintain a clean, safe, and organized environment adhering to infection control protocols
- Respond promptly to patient emergencies or changes in condition
Qualifications
- Valid Licensed Practical Nurse (LPN) license in the province of practice
- Prior experience in nursing home, hospital, acute care, or home health settings preferred
- Knowledge of medical terminology, medication administration, and patient care procedures
- Experience working with diverse populations including seniors, pediatrics, or hospice patients is advantageous
- Strong communication skills and the ability to work collaboratively within a multidisciplinary team
- Ability to handle stressful situations with professionalism and compassion
- Experience in pain management, case management, or dementia care is beneficial
