The Role
We're looking for a registered Occupational Therapist to deliver community-based OT services directly in clients' homes across the Greater Toronto Area. You'll conduct functional assessments, recommend and implement strategies to support independence in daily living, and work alongside our physiotherapy and nursing teams to create truly integrated, client-centred care.
This is a role for an OT who believes that the home is the most powerful setting for meaningful, occupation-focused intervention — and who wants to do that work within a forward-thinking, tech-supported organization.
Key Responsibilities
- Conduct comprehensive occupational therapy assessments in clients' homes, including cognitive, functional, and environmental assessments
- Develop individualized intervention plans focused on enabling independence in activities of daily living (ADLs and IADLs)
- Provide direct OT interventions including ADL retraining, cognitive strategy training, energy conservation, compensatory techniques, and caregiver education
- Conduct home safety assessments and make environmental modification recommendations (grab bars, ramps, bathroom equipment, etc.)
- Recommend, prescribe, and train clients in the use of adaptive equipment and assistive devices
- Write clinical notes, progress reports, and functional capacity documentation in compliance with COTO standards (supported by our AI-assisted voice-to-document drafting tool)
- Collaborate with PTs, nurses, PSWs, and care coordinators to ensure seamless, integrated care
- Communicate with referring physicians, OHAH care coordinators, insurance companies, and family members
- Supervise Occupational Therapy Assistants (OTAs) as applicable
- Educate clients and caregivers on strategies to maintain function and safety at home
What You Bring
- Registration in good standing with the College of Occupational Therapists of Ontario (COTO)
- Experience in community, home health, or geriatric OT practice (preferred)
- Strong assessment skills and familiarity with standardized OT assessment tools
- Experience with home safety assessments and equipment prescription
- Excellent clinical documentation and report-writing skills
- Ability to work independently, prioritize a community caseload, and communicate proactively
- Experience with insurance reporting and third-party billing (OHIP, WSIB, MVA, extended health) is an asset
- Access to a vehicle and a valid Ontario driver's licence
- CPR/First Aid certification
