About the Organization and our Partners:
The South Island Division of Family Practice provides ongoing professional support for Family Physicians with a collaborative and innovative approach to strengthen family practice and patient care in South Island communities.
Divisions of Family Practice are a collaboration between the Ministry of Health and the Doctors of BC. and are community-based groups of family physicians working together to achieve common health care goals. We are a membership-based, not-for-profit society representing Family Physicians in Saanich Peninsula, Western Communities, and Salt Spring Island areas.
Program Lead
Reporting to the Divisional Program Manager, the following shall comprise the key deliverables of the position.
Mission, Vision, and Organizational Alignment
- Support the mission, vision, values, attributes, policies and procedures of the South Island Division including the Division’s broader initiatives such as the South Island Primary Care Networks program.
- Represent and maintain positive relationships with Division members, interest-holders, and community members in a professional manner.
- Abide by the Code of Conduct provided by the South Island Division.
- Demonstrate and promote a commitment to cultural safety and humility.
- Complete the San’yas Anti-Racism Indigenous Cultural Safety training (or show proof of completion upon hiring).
- Collect, manage, store, and use information in compliance with the Personal Information Privacy Act (PIPA).
- Collaborate effectively with colleagues, demonstrating respect for diverse perspectives and ensuring alignment across the team to achieve organizational goals.
- Contribute actively to team meetings, sharing ideas and feedback in a manner that respects others’ viewpoints.
- Collaborate with team members to organize, maintain, and update files and databases, ensuring that information is accessible and up to date.
Role Specific
- Document and provide monthly reports to the Board of Directors, the PCN Steering Committee, and any other relevant committees, working groups, and/or interest-holders as required.
- Foster a supportive and positive work environment that encourages family physicians to build long-term careers within the South Island Division geography.
- Support comprehensive, full scope, project and change management from governance to clinic operations.
- Complete current state needs assessment and team-based care readiness assessments.
- Conduct interest-holder analysis and management, and engage physicians and other healthcare providers to mobilize for change.
- Develop, engage and lead change management strategies and plans for team-based care, incorporating other practice supports available.
- Identify training, tools, and supports to support change.
- Identify issues requiring elevation to higher level decision makers and support resolution and prepare briefing materials as required.
- Act as the primary point of contact for clinics regarding integrating and sustaining team-based care and partnering with Island Health in overseeing the PCN clinical resources.
- Create learning and networking opportunities for team-based care.
- Build knowledge and tools to support the hiring and integration of nursing and allied health clinicians in patient medical homes and facilitate the onboarding, orientation, training, and management process.
- Develop, implement and refine team-based care models in response to evolving primary care needs.
- Ensure collaborative engagement with a variety of multi-interest-holder groups and nurture partnerships with First Nations, Métis, and Inuit partners, Island Health, patient partners and families, physicians, and other primary care providers, allied health professionals, and community organizations.
- Represent the Primary Care Network at various local, regional, and/or provincial meetings, as needed.
- Collaborate in the development of new patient medical home models and innovations in primary care.
- Work on all phases of project management including, but not limited to leading meetings, project planning, privacy impact assessments, evaluation and monitoring of deliverables, presenting to internal and external audiences, and preparing reports.
- Support recruitment, retention and clinic readiness activities for Nurse Practitioners funded through the PCN.
- Prepare and assist in delivering a variety of communication and marketing tools for the South Island Division newsletter, website, posters, etc.
- Travel across assigned areas as required.
- Access to a vehicle is required.
- Other duties as required.
Education, Training and Experience
- Post graduate degree in healthcare, business administration, organizational design, or project management (master’s degree preferred), or an equivalent combination of education and experience.
- At least five (5) years of experience in health care/program leadership.
- Knowledge of the BC healthcare system and experience working in a primary healthcare setting or experience working with physicians and/or allied health care providers preferred.

- Experience engaging with stakeholders and partners.
- Ability to set priorities effectively and problem solve, including early identification of risks.
- Collaborative team style with strong interpersonal communication, relationship building and problem-solving skills.
- Excellent organizational skills and ability to prioritize and manage multiple tasks to meet deadlines with quality output.
- Experience working within or general understanding of Board led and Non-Profit organizations governance and structures.
- Cultural awareness and competency.
- Proficiency in Microsoft Office (Word, Excel, Outlook, PowerPoint, Teams) and Zoom is required.
- Available to attend late morning and evening meetings
Conditions of Employment:
- Full time (37.5 hours per week) - hours determined collaboratively with supervisor.
- Potential work from home hybrid-remote, secure worksite and/or at the South Island Division’s office location in Royal Oak.
- Ability to accommodate evening and early morning meetings is required.
- Ability to travel to meetings and clinics in the communities is required. The successful candidate will have a valid BC driver’s license, access to a reliable vehicle and automobile insurance which includes driving for business purposes.
- Must live in the Greater Victoria Area.
