Overview
We are seeking a dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide compassionate patient care across various settings, including nursing homes, hospitals, assisted living facilities, and home care environments. This role offers an opportunity to work with diverse patient populations, including seniors, children, and those requiring hospice or acute care services. The LPN will play a vital role in delivering high-quality nursing services, ensuring patient safety, and supporting clinical operations through effective communication and documentation.
Responsibilities
- Administer medications and treatments accurately following physician orders and established protocols
- Monitor patient vital signs and observe for changes in condition
- Assist with patient assessments and document findings using electronic health records such as Epic and Cerner
- Provide wound care, phlebotomy, and other clinical procedures as needed
- Support patients with activities of daily living, including mobility and personal hygiene
- Collaborate with interdisciplinary teams to develop and implement individualized care plans
- Educate patients and families about health conditions, medication management, and discharge instructions
- Maintain a safe and clean environment adhering to infection control standards
- Document all patient interactions thoroughly in compliance with healthcare regulations
Requirements
- Valid LPN license in the state of practice
- Prior experience in nursing homes, hospitals, acute care, dementia care, pediatrics, home care, hospice care, or senior living preferred
- Knowledge of medical terminology and medication administration protocols
- Experience with electronic health records systems such as Epic or Cerner is highly desirable
- Strong patient care skills with a compassionate approach to diverse populations including children and seniors
- Ability to perform laboratory procedures such as phlebotomy and laboratory testing when required
- Familiarity with pain management techniques and case management practices is a plus
- Excellent communication skills and the ability to work effectively within a multidisciplinary team
- Commitment to maintaining confidentiality and adhering to healthcare regulations and safety standards
Job Type: Part-time
Pay: From $40.00 per hour
