Are you a compassionate and skilled nurse looking to make a meaningful impact? Join the Fort Smith Primary Care Clinic and play a vital role in delivering high-quality, patient-centred care. Working alongside an interdisciplinary team, you will provide direct nursing care, support health promotion and education, and help improve the health and well-being of individuals, families, and communities in the Northwest Territories.
Key Responsibilities

Provide direct patient care and clinical support in a busy primary care setting.

Perform assessments, vital signs, treatments, and nursing procedures within scope of practice.

Assist physicians and nurse practitioners with examinations and minor procedures.

Coordinate patient follow-up care, referrals, and Telehealth appointments.

Deliver health education and promote wellness and disease prevention.
-
Maintain accurate documentation and uphold patient safety and confidentiality standards.

Collaborate with interdisciplinary team members to deliver seamless, client-centred care.
-
Foster a culturally safe, respectful, and compassionate care environment.

Work flexible hours, including occasional evenings and Saturdays.
Key Requirements:
-
A Licensed Practical Nursing Program, including Medication Administration and participation in continuing education as it pertains to the profession; AND,
-
two (2) years of experience in working in an interdisciplinary model; such as acute care/hospital setting, public health, and medical office/family practice
Additional Requirements:

Must be eligible for registration with the College of Nurses in the Northwest Territories and Nunavut
