Women’s Health in Women’s Hands Community Health Centre (WHIWH-CHC) provides racialized women, trans and non-binary clients from the African, Black, Caribbean, Latin American, and South Asian communities in Toronto and surrounding municipalities with culturally safe, relevant, and responsive primary healthcare. We are changemakers engaged in knowledge transfer and exchange, research, capacity building, and advocacy within and across multiple systems.
WHIWH-CHC is recruiting a highly motivated, professional and dynamic individual to join the Population Health Team.
About the IPCT Initiative
The Interprofessional Primary Care Team (IPCT) Initiative is funded through the Ministry of Health.
The successful candidate will contribute to meeting the goals, deliverables, and performance
expectations of the IPCT Initiative in accordance with funding requirements.
RESPONSIBILITIES:
- Provide trauma-informed, culturally responsive intakes and comprehensive biopsychosocial assessments for clients, utilizing strengths-based, client-centred, anti-oppressive, and evidence informed approaches.
- Deliver brief counselling, supportive counselling, and therapeutic interventions appropriate to client needs, including (but not limited to): system navigation, care coordination, and referrals to internal and external community resources.
- Maintain an active caseload while balancing direct clinical care with community outreach, program development, interdisciplinary collaboration, and population health initiatives.
- Assess and respond to client crises, including risk assessment, safety planning, de-escalation, and appropriate referrals, in collaboration with the interdisciplinary team and community partners.
- Advocate for clients at the individual, community, and systems levels by addressing barriers to care and facilitating equitable access to health, housing, income, legal, financial, social supports, and more.
- Participate in the planning, development, implementation, monitoring, and evaluation of population health initiatives and social work services that address the social determinants of health, reduce health inequities, and improve community well-being through evidence-informed and quality improvement approaches.
- Develop, coordinate, facilitate, and evaluate workshops, psychoeducational sessions, health promotion activities, community-building initiatives, and skill-building programs using trauma informed, client-centred, and culturally responsive practices.
- Engage community members, agencies, and cross-sector partners to strengthen collaborative relationships, support community-led initiatives, and build sustainable support networks for priority populations.
- Collaborate effectively with internal team members including (but not limited to): physicians, nurse practitioners, nurses, therapists, dietitians, health promoters, community health workers, admin team, and external service providers to develop coordinated care plans that address clients' health and social needs.
- Demonstrate knowledge of the social determinants of health, health equity, anti-oppressive and anti-racist practice and cultural humility to effectively support the needs of racialized, newcomer, and other equity-deserving populations. Maintain current knowledge of community resources and services available throughout Toronto and surrounding municipalities.
- Maintain accurate, timely, and comprehensive documentation in the electronic medical record in accordance with organizational policies, professional standards, privacy legislation, and regulatory requirements.
- Participate in interdisciplinary case conferences, chart reviews, and continuous quality improvement initiatives to enhance client outcomes, service delivery, and program effectiveness.
- Participate in outreach, community engagement, research, data collection, and evaluation activities, as required, to support organizational priorities and population health objectives.
- Maintain professional competence through ongoing learning, reflective practice, adherence to the standards of the Ontario College of Social Workers and Social Service Workers (OCSWSSW), and applicable legislation and ethical guidelines.
- Demonstrate professionalism, accountability, initiative, sound clinical judgment, and a commitment to collaborative, client-centred, trauma-informed, and equitable practice.
- Perform other duties as assigned to support the ongoing needs and strategic priorities of the organization.
QUALIFICATIONS:
1. Registered Social Worker and/or has an approved equivalent of education and related experience.
2. Member in good standing with a relevant regulatory body such as the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
3. Demonstrated ability to communicate and collaborate effectively within an interdisciplinary healthcare team, while accepting clinical direction and contributing to shared care planning.
4. Minimum of 1 year experience working within a non-profit, Community Health Centre, primary care, or community-based organization.
5. Advanced understanding and practical application of anti-oppressive, culturally responsive, and client-centred approaches to social work service delivery, including consideration of health promotion and social determinants of health.
6. Experience working with diverse communities, including racialized, newcomer, and other equity deserving populations, and demonstrated knowledge of community resources and services available to priority populations in Toronto and surrounding municipalities.
7. Ability to complete documentation in accordance with the standards and practice guidelines of the OCSWSSW.
8. Demonstrated ability to assess complex client needs, effectively handle client crisis and conflict and establish appropriate professional boundaries
9. Ability to plan, organize, and prioritize workload in a fast-paced environment with minimal supervision while demonstrating initiative, accountability, and sound clinical judgment.
10. Strong interpersonal, written, and oral communication skills, including the ability to facilitate individual and group sessions.
11. Ability to work collaboratively within an interdisciplinary healthcare team while also demonstrating the ability to work independently.
12. Proficiency with computer applications, including Microsoft Word, Excel, PowerPoint, Outlook, and electronic health record/documentation systems.
13. Proficiency in language(s) that reflect the priority communities served is considered an asset.
14. Ability to work flexible hours, including evenings and occasional weekends, to support program and community needs.
15. This position includes on-site work requirements to support team collaboration, client care, and community programming. The on-site schedule will be determined in consultation with the Director, Population Health, based on program needs and operational requirements.
REPORTING:
This position reports to the Director of Population Health.
