JOB SUMMARY:
The Medicine / Surgery and Women & Children's Social worker will assist Windsor Regional Hospital consumers, from admission through discharge, with their psychosocial needs. This care includes specialized assessment, treatment, rehabilitation and community reintegration. This position will improve care of the patients in the hospital and establish linkages with community partners which support and/or care for the patients. In addition this position provides for a unique collaborative relationship with the multidisciplinary team. The Social Worker will provide comprehensive assessment, rehabilitation and treatment to the patients and their families. The Social Worker complies with the Occupational Health and Safety Standards for the hospital and is proactive in promoting a safe work environment. The incumbent is responsible for delivery of care consistent with the goals and objectives of the hospital and the standards of relevant professional organizations. This position contributes to the hospital mission by being a team member who ensures confidentiality, quality, and client-centered health care.
QUALIFICATIONS:
Minimum 1-2 years of discharge planning with the Acute Care patient population (Medical / Surgical, Critical Care, Rehab)
Knowledge of placement options and community services available for the adult patient population
Knowledge of community services available for the Women and Children’s patient population
Social Work knowledge and skills appropriate to the target population
Excellent interpersonal skills
Excellent communication skills, both written and oral
Ability to organize and develop the service in a given area
Ability to work effectively as a member of a interdisciplinary health care team
Commitment to departmental and professional standards and accountability
Participation in Quality Assurance activities
French language proficiency an asset
EDUCATION:
Minimum of Bachelors Degree in Social Work - required;
Masters Degree in Social Work ~preferred;
Relevant internship / placement educational experience (Acute Care)
Registration with the Ontario College of Social Workers - required
