At the Child and Parent Resource Institute (CPRI) we provide highly specialized trauma-informed mental health and developmental services. These services include Assessment, Consultation, Treatment, Research and Education. We work with local community services, building on what they have already done and use evidence based practices to adapt our approach to the unique needs of each child. CPRI is looking for a Case Management Coordinator to apply their case management skills and to provide a consistent, professional point of contact for clients, families, community professional and agencies.
Note: This role requires you to be on site 5 days per week at the Child and Parent Resource Institute in London, Ontario. Travel will be required within the service area.
As the Case Management Coordinator, you will:
- Work collaboratively with caregivers, community professionals and our clinical team
- Coordinate the client services processes and completing assessment tools
- Chair and organize case meetings with multidisciplinary teams
- Provide resources to support client and caregiver needs
- Ensure treatment recommendations are communicated to caregivers and community agencies
Successful completion of:
- a Social Services Worker, Child Care Worker or Developmental Services Worker program, or
- equivalent combination of experience and education in the treatment of children with mental health and/or developmental needs
- You know children's mental health and services systems, behavioural principles, and possess clinical skills
- You know relevant psychological theories, legislation, and best practices to apply a range of treatments including child behaviour assessments
- You have a clear understanding of developmental and mental health disorders to assess, manage, and implement treatment plans/interventions
- You know or can acquire knowledge of social services in Ontario, specifically related to children services
- You can prepare reports accurately and concisely
- You collaborate effectively with a variety of stakeholders such as professionals, staff, the public, and community representatives
- You ensure recommendations from treatments are documented and communicated to clients' caregivers and community agencies
- You can gather relevant information to identify treatment needs and participate in research initiatives.
- You have organizational skills to coordinate and manage information and data from a variety of sources
- You can plan and prioritize workload and work within established timelines
- You are proficient with computer software such as MS Word, Excel, and Outlook
- You can work independently and make decisions under the general guidance of the treatment team.
