We are seeking a dedicated and compassionate Registered Practical Nurse (RPN) to join our healthcare team. The ideal candidate will provide high-quality patient care across various settings, including nursing homes, hospitals, assisted living facilities, and home care environments. This role offers an opportunity to work with diverse patient populations, including seniors, children, and individuals requiring specialized care such as dementia, hospice, or pain management. The RPN will collaborate with interdisciplinary teams to deliver safe, effective, and personalized nursing services while adhering to all regulatory standards and best practices.Responsibilities
- Administer medications and vaccinations accurately in accordance with physician orders and established protocols.
- Monitor the patient's vital signs and assess health status regularly to identify any changes or concerns.
- Provide comprehensive patient care tailored to individual needs, including dementia, hospice, pediatric, or senior care.
- Document patient information accurately using electronic health records such as the Accuro EMR system.
- Educate patients and families about health conditions, medication management, and post-discharge care plans.
- Collaborate with physicians to coordinate effective care plans.
- Maintain a safe and clean environment by following infection control protocols and safety guidelines.
- Support case management activities by tracking patient progress and facilitating appropriate resource utilization
Experience
- Prior experience working in nursing homes, hospitals, or assisted living facilities is highly preferred.
- Experience in acute care settings or home care environments is advantageous.
- Familiarity with medical terminology and medication administration procedures is essential.
- Proficiency with electronic health record systems, such as Accuro EMR, is MANDATORY.
- Strong patient care skills with the ability to work effectively with diverse populations, including children and seniors
- Previous experience in case management or medical office settings will be considered an asset.
