The Case Manager is responsible for the provision of individual and group supports for independent living (supportive counselling, life skills training and case management) to those with serious mental illness, or those at risk.
KEY DUTIES - RESPONSIBILITIES
Ø Assess client’s immediate and ongoing needs
Ø Assist clients to identify personal strengths and develop treatment goals
Ø Provide information and strategies for health promotion
Ø Assist clients to identify and advocate for their civil and legal rights
Ø Facilitate access to other services in areas such as assistance with daily living, crisis intervention, and treatment
Ø Provide counseling and information to reduce the incidence, duration and intensity of a crisis
Ø Work collaboratively with other services, supports and resources, with respect to diversity
Ø Assist clients to identify, build and maintain a natural support system
Ø Evaluate the achievement of goals and client satisfaction
Ø Review and revise treatment plans with clients to ensure the provision of relevant services
Ø Provide counseling using various therapeutic modalities
Ø Provide treatment planning using internal and external consultation as appropriate
Ø Ensure high quality assessment and treatment of clients
Ø Ensure adequate referral when needed
Ø Participate in regular staff meetings and contribute to in service activities
Ø Collaborate in the multi-disciplinary approach to service
Ø Meet professional standards according to the relevant professional code of ethics
Ø Accurately and fully maintain statistical reports and case records according to program and organizational standards
Ø Comprehensive individualized assessment and planning
Ø Consultation and co-ordination with the health care system
Hours of work are 37.50 per week. The normal workday is 8 hours, including a 30-minute unpaid lunch break. Working in a busy office environment with frequent interruptions. Some evening and weekend work may be required. Mild to moderate physical activity, including the provision of practical assistance to clients in household management and maintenance tasks and participation in a variety of recreational activities.
Frequent travel within the Grey-Bruce catchment area

What you will bring
Ø A university degree in a relevant program
Ø Experience working with clients with serious mental illness and/or addictions
Ø Minimum of three years recent and relevant experience in adult community mental health services (supportive housing, case management)
Ø Demonstrated ability to work well with peers, colleagues from other agencies, and with various consumer and stakeholder groups
Ø Training and experience related to concurrent disorders is an asset
Ø Sound judgement, particularly with respect to high risk or sensitive situations
Ø An understanding and appreciation of rural communities
Ø Demonstrated knowledge and skill in assessment, psychosocial rehabilitation, crisis intervention and counselling (including motivational interviewing and solution-focused therapy)
Ø Familiarity with treatment principles and techniques that apply across the continuum of psychosocial rehabilitation and mental health services, including case management.
Ø Awareness of mental health services, resources and agencies within Grey/Bruce County
Ø Ability to work both independently and as a member of a multi-disciplinary team
Ø Knowledge of psychiatry, psycho-social rehabilitation, mental health policy, and DSM V
Ø Excellent communication, interpersonal and networking skills
Ø Effective decision making and crisis intervention skills
Ø A valid Canadian driver's license and the use of a reliable personal vehicle
Ø Minimum $2,000,000 third party liability vehicle insurance
Ø A satisfactory police record check including vulnerable sector screening
