Job Summary
Four Counties Addiction Services Team (Fourcast) is a community-based addiction treatment service provider that strives to provide the highest quality of evidence-based addiction treatment and case management services. Fourcast offers services across 5 sites in the region. The community treatment program staff work in-office and community settings and are often co-located and integrated with other organizations to reduce barriers for those accessing services.
Four Counties Addiction Services Team (Fourcast) is pleased to partner with various Peterborough agencies, to offer services through our new Peterborough Homelessness Addiction and Recovery Team (HART) Hub. The Peterborough HART Hub is designed as a low-barrier, trauma-informed, culturally responsive, health care model that integrates primary care, mental health, substance use treatment, housing with supports, and health systems navigation.
Reporting to the HART Hub Program Manager, the After Care Addiction Counsellor, is responsible for the overall service delivery of HART Hub’s After Care Transitional Housing Program. The program offers transitional supportive housing, individual counselling, group counselling, psychoeducational groups and supportive housing skills development using a variety of applied treatment modalities to support substance use and homelessness recovery.
The 6-month program offers individuals and opportunity to explore and support their goals related to substance use, mental health and housing stability. The Program includes a combination of structured, scheduled activities and unstructured time seven days per week in their home setting under an occupancy agreement. The program will assist individuals to develop skills to maintain goals of substance use abstinence and long-term housing stability.
About The Position...
This is a full time position working on a rotational schedule consisting of one week of day shifts and one week of evening shifts.
This position will have the unique opportunity to take a leadership role to assist in the development of program content and processes. Your clinical expertise and experience will contribute to developing a program that supports positive outcomes for those who seek substance use recovery and housing stability through this program.

What You'll Do as a Hart Hub Addiction Counsellor:
- Provide individual and group addiction counselling sessions
- Develop collaborative personalized recovery and housing stability plans
- Support women navigating substance use recovery, mental health challenges, physical health and life transitions
- Foster a safe, empowering, and trauma-informed environment
- Collaborate with community partners and support services
- Help residents build confidence, skills and successful coping strategies
- Be a key part of a multidisciplinary team committed to lasting recovery outcomes

What we're looking for:
Education:
- Bachelor of Social Work, Psychology, or related degree, with a postgraduate certificate in Addictions and Mental Health, or an equivalent combination of work experience and education.
Experience:
- Minimum 3-5 years’ experience working in a related position preferred, ideally in a Bed-Based addiction treatment setting.
- Advanced skills in addictions interventions including counselling/therapy, crisis intervention, advocacy, mediation, and conflict resolution.
- Demonstrated experience working with people with serious mental illness and complex personal needs.
- Experience using evidence-based screening and assessment tools.
- Background or training in special education is a plus, demonstrating adaptability for diverse client needs.
- Compassionate demeanor with a commitment to supporting clients through sensitive transitions in their recovery journey.
Certifications, Licenses and Records:
- Valid First Aid/CPR
- Non-Violent Crisis Intervention (NVCI) certificate.
- Certification in GAIN assessment would be an asset.
- Applied Suicide Intervention Skills Training (ASIST) an asset.
- Food Handler’s certificate an asset.
- Satisfactory Vulnerable Sector Check is required.

Pay: $34.53-$40.41 per hour
