Maple Family Health Team is seeking a Primary Care Registered Practical Nurse to join our Amherstview Medical Clinic. In this role, you will provide comprehensive care to patients of all ages while working alongside family physicians and an experienced interdisciplinary team dedicated to improving the health of our community.
About Maple Family Health Team
Since 2007, Maple Family Health Team has provided interdisciplinary primary care services to patients throughout the Kingston area. Our nurses and allied health professionals work collaboratively with family physicians and one another to deliver accessible, comprehensive and patient-centered care.
What you will do
As a Registered Practical Nurse, you will
· Provide compassionate nursing care to patients across the lifespan, from infants to older adults.
· Assess patient needs and support physicians in delivering comprehensive primary care.
· Deliver health teaching and promote disease prevention and healthy lifestyle practices.
· Administer immunizations and perform nursing procedures including ear irrigation, suture removal, EKGs, BP Tru and Holter monitor placement.
· Support chronic disease management and preventative care initiatives.
· Accurately document assessments, interventions and patient education within the electronic medical record.
· Contribute to quality improvement initiatives and support Maple Family Health Team programs and services.

· has a current, unrestricted registration with the CNO as a Registered Practical Nurse

Additional information about the position
· Permanent, part-time schedule (4 days per week)
· Immediate enrolment in the Healthcare of Ontario Pension Plan (HOOPP)
· Health and Dental benefits begin after 4 months of employment
· Vaccination requirements: Full COVID-19 vaccination and proof of immunity to Measles
