Memory Care Wellness Coordinator (RPN) – Full-Time
Our employer partner in the Senior Living and Retirement Residence sector is seeking an experienced and compassionate Registered Practical Nurse (RPN) to join a specialized Memory Care team. This leadership-focused role supports residents living with dementia and cognitive impairment through comprehensive assessments, individualized care planning, medication oversight, care coordination, and team leadership.
The ideal candidate is passionate about person-centered dementia care and has experience supporting older adults in retirement living, long-term care, memory care, or related healthcare settings.
Key Responsibilities:
- Conduct comprehensive resident assessments and develop individualized care plans.
- Coordinate and oversee the delivery of memory care services in collaboration with residents, families, healthcare providers, and interdisciplinary teams.
- Monitor resident health status and respond appropriately to changes in condition.
- Oversee medication administration practices and ensure compliance with professional nursing standards.
- Maintain accurate clinical documentation and electronic health records.
- Lead care conferences and provide education and support to residents, families, and team members.
- Support quality assurance, infection prevention, and regulatory compliance initiatives.
- Mentor and provide guidance to care staff to promote consistent, high-quality resident care.
- Participate in emergency response planning, wellness programs, and continuous quality improvement activities.
Qualifications:
- Current Registered Practical Nurse (RPN) registration in good standing with the College of Nurses of Ontario.
- Minimum 3 years of nursing experience in retirement living, long-term care, memory care, dementia care, or a related seniors' healthcare environment.
- Experience supporting older adults living with dementia, Alzheimer's disease, or cognitive impairment.
- Strong clinical assessment, care planning, and medication management skills.
- Current CPR certification.
- Experience with electronic health records and clinical documentation systems.
- Excellent communication, leadership, and organizational skills.
- Valid driver's license, insurance, and access to a reliable vehicle.
- Ability to work flexible schedules, including weekends and occasional call-ins.
Compensation:
- $30.00 – $35.00 per hour, based on experience.
