PALLIATIVE CARE CLINIC AND TRIAGE NURSE
(Part-Time – 4 days/week or 30 hours/week)

We are looking for someone to make a difference in the lives of individuals living with a life-limiting illness. If you are passionate about providing quality compassionate care that supports the physical, emotional, spiritual, and psychosocial needs of individuals and families copying with a palliative illness, we want to hear from you.
We have an immediate opening for a Palliative Care Clinic & Triage Nurse to coordinate clinic referrals, process referrals, arrange appointments and meet people on the day of their appointment. You will triage referrals to determine a clinic visit or home visit by a palliative physician and conduct nursing assessments to ensure timely access to care.
Key Responsibilities
· Intake and assessment of patient referrals for clinic and home visits
· Triage/assess urgency of referrals ad contact referring provider and patient/family to determine needs and connect to appropriate service/care
· Flag and communicate urgency to palliative physicians as appropriate
· Contact individuals to confirm referral information, complete initial assessments, and identify priority concerns
· Patient and family education
· Administrative duties to support efficient functioning of the clinic including arranging appointments, patient follow-up, reminders, maintaining patient information, etc.
Qualifications
· Registered Nurse licensed with College of Nurses of Ontario
· Three (3) years of experience in palliative care
· Highly proficient clinical assessment and triage skills
· A police Vulnerable Sector Check is required as part of employment
· Proof of full COVID19 vaccination (or accommodation request under a protected ground, if applicable)
· Demonstrated excellent communication/interpersonal skills to work collaboratively with physicians, partner agencies and families
· Valid driver’s licence and access to a reliable vehicle with insurance coverage for business purposes is an asset
