CPH is also the Network Lead Organization for the Ontario Structured Psychotherapy (OSP) Program in Brampton, Halton and Mississauga. OSP offers adults (18+) in Ontario free, short-term, evidence-based cognitive-behavioural therapy and related services for depression, anxiety, and anxiety-related conditions.
What is this role?
We are looking for a Registered Practical Nurse (RPN) who is compassionate and will support individuals, families, groups, and communities to enhance their individual and collective wellbeing. The ideal candidate requires a thorough knowledge of primary care nursing and the ability to function effectively in a team environment with physicians, nurse practitioners, registered nurses, allied health professionals and administrative staff. The role also requires a high level of interpersonal skills to interact with patients and their families. In addition, the RPN possesses and utilizes the knowledge, skill, and judgment to function autonomously within the RPN full scope of practice while collaborating with other members of the multidisciplinary team to ensure optimal health outcomes. All RPNs at CarePoint Health are involved in a number of clinical pathways and programs. These programs often involve coordination of clinical care.
Responsibilities:
Reporting to the Clinical Manager, your key responsibilities would include:
- Provide patient oriented, goal directed nursing care in accordance with the standards of care from the College of Nurses of Ontario
- Support the nurse practitioners and onsite physician (s) in daily clinics; perform nursing interventions such as taking vital signs, applying aseptic techniques including sterile dressing, specimen handling, and point of care testing; perform prescribed clinical services such as phlebotomy.
- Administer immunizations and other medications to all age groups; monitor vaccine supply and order vaccines as needed.
- Triage incoming patient connections and calls. Use knowledge, skill and judgment to assess patient’s health needs, discuss test results and respond to questions pertaining to health promotion and disease prevention.
- Provide direct nursing and coordination support to various programs and care pathways at CPH including (but not limited to) vaccination clinics, chronic disease management, spirometry testing, physician attachment, memory clinic, health education groups and other programs as they are developed.
- Maintain accurate, concise, and confidential EMR (Accuro) documentation pertaining to patient history and encounter data.
- In addition to the EMR, perform data entry utilizing digital health tools that are required to support clinical care.
- Communicate ideas and concerns to the health care team and participate in problem solving.
- Provide mutual support to other nursing staff within CPH, the allied healthcare team, the health promoter, and other staff members.
- Demonstrate an understanding of chronic disease management and patient self-care strategies.
- Maintain that all medical information provided is current and unbiased.
- Assist patients and their families to identify resources which lead to the maximum state of health.
- Collaborate with other members of the health care team to develop standards and routines for care of patients; plan, organize and determine priorities in the provision of patient care.
- Active involvement in the overall coordination of patient care; Initiate appropriate action in the response to changing patient needs.
- Participate in staff meetings and review current organizational operations.
- Function as a clinical resource for administrative staff, and other disciplines; support clinic operations; undertake triage responsibilities.
- Flexibility to work evenings and weekends as required.
- Ability to travel to satellite sites or provide home visits as needed.
- Other duties as assigned.
Qualifications:
- Minimum 3 years nursing experience, primary care experience strongly considered an asset
- Current certificate of registration from the College of Nurses of Ontario
- Excellent patient assessment and triaging skills
- Knowledge of Immunization guidelines in Ontario
- Knowledge of and working experience with EMR systems and applications
- Excellent verbal and written communication skills
- Ability to work collaboratively as a member of a multi-disciplinary team and liaise with other agencies and service providers
- Able to respond to crisis situations with demonstrated critical thinking
- Basic Cardiac Life Support certification
- Advanced certification (such as Asthma Educator, Foot Care, Breastfeeding, Pediatrics Assessment, Wound Management etc.) is considered an asset
- Competent skill sets in advocating on behalf of clients, interdisciplinary team collaboration, group facilitation support, consultation with professional colleagues and caregivers/families
- Be enthusiastic, passionate and self‐driven who will participate in data management and measurement of quality improvement indicators of patient outcomes;
- Knowledge and proficiency in the provision of crisis intervention, including risk assessment and management form a non-medical perspective, is essential
- Knowledge of community and the resources available considered an asset
- Experienced working with at-risk/vulnerable population
- Be sensitive to patients’ issues and needs and have the capacity to address these issues
- Openness to learning and using new digital tools and platforms
- Contribute to program development
- Ensure all communications reflect CPH standards and facilitate a positive patient, family, and provider experience
- Uphold CPH values and be an active contributor to a positive, innovative, collaborative, and cooperative work environment
- Good reading and writing communication skills in English, a second language would be an asset
- Acts as a professional and knowledgeable role model or mentor to colleagues and students; and
- Maintain confidentiality in the workplace and within the community.
- A satisfactory and current Criminal Background and Vulnerable Sector Screening and proof of immunization required
- Vaccines (COVID-19 and others) are a requirement of the job unless you have an exemption on a medical ground pursuant to the Ontario Human Rights Code
