Position: Registered Practical Nurse – Unattached Care Clinic and/or Primary Care Support
1.0 FTE, Permanent with Benefits

Position Summary:
The Registered Practical Nurse (RPN) is a member of the Couchiching Family Health Team (CFHT), and an integral part of an interdisciplinary health care team. The Registered Practical Nurse (RPN) will deliver practical nursing care in accordance with CFHT policies and professional practice standards.
Working collaboratively with Nurse Practitioners and Primary Care Providers, the RPN delivers patient-centred care to individuals with episodic and ongoing needs, including both unattached patients and those rostered within primary care. This role supports Ontario Health’s Primary Care for All initiative by improving access to care and promoting continuity across the patient population.
The RPN practices within an equity-informed, anti-oppressive framework to ensure inclusive, respectful, and responsive care for individuals experiencing systemic barriers.

Key Responsibilities:
- Provide comprehensive primary care within scope of practice, including assessment, triage, monitoring, and follow-up
- Deliver nursing care in collaboration with physicians, Nurse Practitioners, and interdisciplinary team members
- Administer medications, treatments, and immunizations per CFHT medical directives
- Perform clinic procedures within scope of practice and/or advanced training, including Pap testing
- Support patient access through triage of referrals and incoming calls, including timely response and follow-up
- Contribute to the development and implementation of individualized care plans, including coordination of referrals to internal and community services
- Provide health education, chronic disease management support, and health promotion to patients and families
- Maintain accurate and timely documentation within the electronic medical record (EMR)
- Contribute to quality improvement initiatives, program development, and reporting requirements
- Participate in team meetings and support effective interdisciplinary collaboration
- Provide care within an equity-informed, culturally responsive, and anti-oppressive framework
Qualifications:
- Practical Nursing Diploma and current registration in good standing with the College of Nurses of Ontario (CNO)
- Minimum 2–3 years of relevant nursing experience in primary care, community, or acute care
- Demonstrated assessment, triage, and clinical decision-making skills
- Experience working within an interdisciplinary team environment
- Proficiency with EMRs and standard computer applications
- Current CPR certification (BCLS an asset)

Other Requirements:
- Valid Ontario driver’s licence and access to a vehicle
- Satisfactory vulnerable sector check
- Ability to work flexible hours, including evenings and weekends as required
- Compliance with required immunizations in accordance with public health guidance
- Adapt to the staffing needs of the CFHT by assisting with Nursing responsibilities in other CFHT programs and services as required
