Wellness Coordinator (RPN) – Full-Time
Our employer partner in the Senior Living and Retirement Residence sector is seeking a dedicated and experienced Registered Practical Nurse (RPN) to join their Wellness Team as a Wellness Coordinator. This leadership-focused role supports the delivery of high-quality resident care through assessments, care planning, medication management, care coordination, and regulatory compliance.
Key Responsibilities:
- Conduct resident assessments and develop individualized care plans.
- Coordinate care services and collaborate with residents, families, physicians, pharmacies, and external healthcare providers.
- Administer medications and ensure compliance with professional nursing standards and regulatory requirements.
- Monitor resident health status and respond to changes in condition.
- Lead and participate in care conferences, wellness initiatives, and health promotion programs.
- Maintain accurate clinical documentation and health records.
- Support quality assurance, infection prevention, and regulatory compliance activities.
- Provide guidance, mentorship, and oversight to care staff as required.
- Participate in emergency response, resident admissions, and wellness-related programs.
Qualifications:
- Current Registered Practical Nurse (RPN) registration in good standing with the College of Nurses of Ontario.
- Minimum 3 years of nursing experience in retirement living, long-term care, seniors' care, or a related healthcare environment.
- Strong clinical assessment, care planning, and medication administration skills.
- Experience working with older adults and interdisciplinary healthcare teams.
- Current CPR certification.
- Proficiency with electronic documentation systems and Microsoft Office applications.
- Valid driver's license, access to a reliable vehicle, and appropriate insurance.
- Ability to work flexible schedules, including weekends and occasional call-ins.
Compensation:
$30.00 – $35.00 per hour, based on experience.
