Job Summary
We are seeking a dedicated and compassionate Nurse Practitioner to join our healthcare team. This role involves collaborating with physicians and other healthcare professionals to deliver high-quality medical services, manage acute and chronic conditions, and support patient well-being. Experience with medical terminology, medication administration, and electronic health records such as Epic and Cerner is essential.
Responsibilities
- Conduct thorough patient assessments, including history taking and physical examinations across various age groups
- Diagnose illnesses and develop treatment plans in collaboration with physicians
- Administer medications and perform procedures such as phlebotomy with precision and adherence to safety protocols
- Manage chronic conditions, including pain management and dementia care
- Provide patient education on health maintenance, medication usage, and disease prevention
- Document all patient interactions accurately using electronic health record systems like Epic and Cerner
- Coordinate care with specialists, laboratories, pharmacies, and community resources for comprehensive case management
- Maintain compliance with healthcare regulations and standards while ensuring patient safety at all times
Experience
- Familiarity with medical terminology, medication administration protocols.
- Proficiency with electronic health records systems such as Epic and Cerner is preferred
- Strong case management skills with the ability to coordinate multidisciplinary teams effectively
- Excellent patient care skills demonstrating empathy, professionalism, and attention to detail
