Telemedicine / Primary Care Registered Practical Nurse (RPN)

The Minto-Mapleton Family Health Team (MMFHT) is an interdisciplinary primary care organization serving over 15,000 patients across the rural communities of Drayton, Palmerston, Clifford, and Harriston. Our team includes physicians, nurse practitioners, nurses, social workers, dietitians, and other allied health professionals working collaboratively to support health promotion, chronic disease management, and accessible, patient-centred care.

We are seeking a skilled and adaptable Registered Practical Nurse (RPN) to join our team in a role combining telemedicine and in-person primary care. This role is jointly shared with the Mount Forest Family Health Team.
About the Role
In this role, you will:
- Provide telemedicine services using platforms such as the Ontario Telemedicine Network (OTN) and other digital health platforms
- Provide and support in-office primary care across Minto-Mapleton and surrounding communities
- Collaborate with an interdisciplinary team to deliver coordinated, high-quality patient care
- Participate in patient assessments, follow-ups, and chronic disease management
- Help patients navigate and access care through both virtual and in-person channels
- Support the implementation of quality standards, clinical protocols, and medical directives across sites
This is a dynamic role ideal for a nurse who is a self-starter, enjoys taking a leadership role and is comfortable working with digital health tools while maintaining strong patient relationships in a rural primary care setting.

Qualifications
- Current registration as an RPN in good standing with the College of Nurses of Ontario
- 5+ years of nursing experience
- Experience in primary care and/or telemedicine is considered an asset
- Experience with electronic medical records, particularly PS Suite is considered an asset
- Strong technical skills and confidence troubleshooting digital platforms
- Excellent organizational and communication skills
- Valid driver’s licence and access to a reliable vehicle (travel between sites required)
- Up-to-date vaccinations, including COVID-19
