Position Summary
The Nurse Practitioner (NP), will work as an autonomous health professional with advanced clinical skills to provide direct clinical care, education, consultation and treatment in the Health First Program, support and collaborate with the wound care team, and support outpatient programs within the organization. The NP role will encompass the full scope of advanced nursing practice.
The care delivery model is based on Collaborative Practice which includes both independent and cooperative decision-making, and is based on the preparation and ability of the Nurse Practitioner (NP).
The Health First department is currently seeking a Permanent Full Time Nurse Practitioner to join their team.

Department/Unit/Program Description
Health First is a Chronic Disease Prevention & Management program at Ross Memorial Hospital that offers a variety of outpatient services to assist clients and families living with a chronic disease.
Essential Qualifications
- Current registration with the College of Nurses of Ontario as an Extended Class RN (EC) and is a member in good standing
- Current membership in the Nurse Practitioner Association of Ontario (NPAO) is required
- Current BCLS upon hire is required
- Minimum three (3) years recent clinical experience in advanced practice nursing within a variety of clinical settings
- Certified Diabetes Educator or willingness to complete within two years of date of hire
- Demonstrated performance within the current evidence-based guidelines for managing diabetes and the Canadian Diabetes Standards
- Strong understanding of the impact of chronic diabetes and/or chronic disease on the patient and family
- Advanced knowledge of the nursing process, the consultation process, discharge planning and community resources
- Strong self motivation and passion for program growth and development
- Demonstrated ability to work collaboratively with the Interdisciplinary and Physician Team
- Demonstrated ability to communicate (highly developed written and verbal communication) and work effectively with the multi-disciplinary team and the members of the community
- Effective interpersonal and customer services skills
- Participation in the College of Nurse's Quality Assurance Program
- Recent experience, skill and knowledge in adult teaching theories
- Demonstrated high level organization and time management skills
- Demonstrated knowledge of Microsoft Office applications (Word, Excel, and PowerPoint)
- Expertise in developing and maintaining excellent relationships with a broad range of individuals and partner organizations
- Demonstrated ability to work independently and efficiently
- Demonstrated ability to meet the attendance standard set by the Ross Memorial Attendance Awareness Policy
- Must have a valid driver's license and access to a reliable vehicle
- Demonstrated commitment to professional development and continuing education

- Epic trained or ability to train and pass testing

Preferred Qualifications
- Membership with the Registered Nurses Association of Ontario (RNAO)
- Nursing or other related research experience
- ACLS
- Advanced wound assessment skills, complex wound course
Hours of Work
Currently, must be available for eight (8) hour day shift Monday to Friday. Shifts and hours may vary according to departmental requirements and as per the ONA Collective Agreement. Travel may be required.
