Position Summary
The Residential Support Worker provides safe, respectful, person-centred support to individuals living in a Lyra Support Services residence. Individuals supported may have developmental disabilities, autism, mental-health needs, dual diagnoses, behavioural challenges or other complex support needs.
The Residential Support Worker helps individuals develop independence, participate in their community, maintain their health and safety, and achieve the goals outlined in their Individual Support Plan, Behaviour Support Plan and other care plans.
Key Responsibilities
Direct Support and Personal Care
- Provide respectful, person-centred support based on each individual’s needs, preferences, abilities and goals.
- Assist with personal hygiene, dressing, bathing, toileting, grooming and other activities of daily living when required.
- Support individuals with meals, nutrition, household routines and maintaining a clean and safe living environment.
- Encourage independence and provide only the level of assistance required.
- Support individuals with community activities, appointments, recreation, education, employment and family visits.
- Promote choice, dignity, privacy and informed decision-making.
Behavioural and Emotional Support
- Follow all Behaviour Support Plans, Safety Plans, Crisis Plans and approved clinical recommendations.
- Recognize early signs of distress or escalation and respond using proactive, trauma-informed and least-restrictive strategies.
- Use verbal de-escalation, redirection, emotional-regulation strategies and approved UMAB techniques.
- Participate in physical containment only when properly trained, authorized and required as a last resort to address an immediate risk of harm.
- Maintain a calm, professional and non-judgmental approach during challenging situations.
- Accurately document behavioural incidents, interventions and outcomes.
Health and Medication Support
- Administer medications according to Lyra policies, physician directions and the individual’s Medication Administration Record.
- Document medication administration accurately and immediately report medication errors or concerns.
- Monitor individuals for changes in physical health, mental health, behaviour or functioning.
- Attend medical, dental and clinical appointments when assigned.
- Follow medical recommendations and communicate relevant information to management.
- Respond appropriately to medical emergencies and seek assistance when required.
Documentation and Communication
- Complete daily notes, communication logs, medication records, incident reports, ABC data, goal tracking and other required documentation.
- Ensure documentation is factual, objective, accurate and completed before the end of the shift.
- Report incidents, allegations, injuries, medication errors, safety concerns and significant changes to the appropriate manager immediately.
- Maintain confidentiality in accordance with Lyra policies and applicable privacy legislation.
- Participate in shift changes and communicate relevant information to incoming staff.
- Attend staff meetings, training sessions, case conferences and supervision meetings.
Pay: $21.00-$25.00 per hour
Expected hours: 1.0 – 44.0 per week
