Job Description:
Position Overview:
Reporting to the Manager of the Centre For Complex Diabetes Care, the Registered Practical Nurse will be responsible for providing excellent care alongside an interprofessional team to a diverse patient population within a multi-service, based Nephrology program. The goal within the MCKC program is to slow progression of Kidney Disease such that patients never go onto dialysis or for patients whose disease is progressing, readying for renal replacement therapy is a key function of this program. Hence, the patient population includes those with various stages and causes of Chronic Kidney Disease with specific care plans that include specific needs for Glomerulonephritis (GN), Polycystic Kidney Disease (PKD), Dialysis Access, and Conservative Care as examples. Kidney transplantation, assessment, education, coordination of transplantation to interested suitable patients and families is also included as renal replacement therapy options to this patient population.
Responsibilities:
The Centre For Complex Diabetes Care RPN will be performing duties including but not limited to:
- Performs clinical assessments for clinic patients
- Provides follow up telephone calls, virtual assessments, and consultations with patients and/or family members as necessary
- Provides support to the interprofessional team members in delivering and coordinating care within the clinic
- Works closely with the Primary registered nurse in clinic to provide optimal patient care and continuity
- Liaise with physicians as needed to ensure patients/families have appropriate prescriptions, lab requisitions, and follow up appointments.
- Coordinate with clerical staff to ensure patients have timely access to care as per ORN guidelines
- Maintain clean working environment and disinfect room as per IPAC practices after each patient.
- Maintains professional development by attending or participating in seminars and conferences relevant to the care of the patient with chronic kidney disease and participates in ongoing continuing education initiatives
- Maintains a current knowledge base regarding evidenced-based practices for the patient with chronic kidney disease
Requirements:
- Current registration as a Registered Practice Nurse with the College of Nurses of Ontario (CNO)
- Current BCLS
- Familiar with the principles of adult education and has demonstrated the application of those principles in practice
- Within the last year, has experience practicing in renal nursing caring for patients with progressing chronic kidney disease, on hemodialysis, on peritoneal dialysis and/or those with a kidney transplant,
- Work with patients living with kidney disease, and foundational knowledge of renal replacement therapies
- Ability to use the nursing process (assessment, planning, implementation, evaluation) in conjunction with her/his basic and ongoing knowledge to arrive at individualized care planning and delivery
- Advocate for patient needs while respecting the patients goals of care
- Identify and recognize abnormal or unexpected patient responses and take appropriate action
- Ability to simultaneously manage multiple nursing interventions
- Strong interprofessional and communication skills
- Computer and communication technology skills (telemedicine)
- Minimum of 5 years of nephrology nursing experience
- Demonstrated excellent problem-solving, decision making and critical thinking skills
- Demonstrated excellent organizational, time-management and team building skills
