Summary:
This is an exciting leadership opportunity to support and advance Health Transformation within Chippewas of the Thames First Nation. The Associate Director of Care will play a key role in shaping and strengthening care delivery across the Health Department, supporting a child-centric, culturally grounded, and community-driven model of care. This role will lead care delivery teams and work closely with the Health Director and Associate Director of Administration & Finance to support a growing department of over 20 staff.
We are seeking a Registered Nurse leader who is passionate about health, grounded in Anishnaabe values and ways of knowing, and committed to building integrated, wrap-around services that support children, youth, families, and community wellness.
Responsibilities:
· Provide senior leadership and operational oversight for care delivery services within the Health Department.
· Lead implementation of a child-centric, family-centred, and culturally grounded model of care.
· Supervise and support supervisors, leads, and frontline care staff.
· Strengthen coordination across programs to improve continuity of care for children, youth, families, adults, and Elders.
· Support development of case management and cross-functional care coordination processes.
· Oversee care delivery practices, service standards, and quality improvement initiatives.
· Support implementation and improvement of centralized electronic medical records and intake processes.
· Ensure compliance with accreditation, clinical, privacy, and reporting requirements.
· Monitor service trends and recommend improvements to care delivery and operations.
· Support staffing models, service planning, and operational improvements.
· Collaborate with CYFWB, Education, and other partners to improve outcomes for children and families.
· Promote culturally safe, trauma-informed, and strengths-based approaches to care.
Qualifications:
· Current registration with the College of Nurses of Ontario as a Registered Nurse.
· Bachelor of Science in Nursing (BScN) or equivalent.
· Minimum 5 years of progressive experience in health services leadership or coordination.
· Minimum 3 years of supervisory or management experience.
· Experience in community health, primary care, or Indigenous health service delivery is preferred.
· Strong understanding of child-centred, family-centred, and culturally grounded care approaches.
· Experience with quality improvement, accreditation, and service planning.
· Experience with EMR systems and clinical documentation.
· Experience working in or alongside a First Nation or Indigenous organization is strongly preferred.
· Equivalent combinations of education and experience may be considered.
