Job Description: LPN – Care Manager and Supervisor

POSITION SUMMARY:
This role involves assessing, planning, implementing, and evaluating nursing care in accordance with nursing competency guidelines and legislative scope of practice. The LPN Care Manager / Supervisor will ensure the highest standard of care is delivered to clients and contribute to the overall operational success of the team.
RESPONSIBILITIES:
Client Assessment and Implementation of Home Support Services:
- Assess and monitor client’s strengths, limitations, and holistic needs (physiological, psychological, socio-cultural, and spiritual).
- Collaborate in developing and modifying individualized care plans and delegate tasks accordingly. Provide training and quality assurance.
- Liaise with the health authority and interdisciplinary team to ensure optimal client outcomes.
- Conduct monthly client monitoring and satisfaction follow-up visits and calls, as needed.
Teamwork and Collaboration:
- Work closely with service coordination and scheduling teams to ensure efficient operations.
- Collaborate with other care managers, providing guidance, support, and leadership in team-building, scheduling, and resource management.
- Actively participate in project/program development and implementation as part of the team.
Marketing:
- Actively engage with the community to promote Living Well as a high-quality home care provider.
Occupational Health and Safety:
- Participate in Health and Safety initiatives, offering suggestions for improvements to workplace safety.
Infection Prevention and Control:
- Contribute to infection prevention and compliance efforts within the organization.
Leadership:
- Lead regular team meetings for Community Health Workers.
- Provide ongoing feedback and formal performance appraisals for Community Health Workers regarding outcomes, professional growth, and person-centered care development.
REQUIREMENTS:
- Graduation from a recognized Licensed Practical Nursing program.
- Full, current licensure with the British Columbia College of Nurses and Midwives (BCCNM).
- Knowledge of current trends in gerontological nursing.
- Minimum 1-2 years of leadership or supervisory experience.
- Experience in home support or community health care preferred.
- CPR certification (or willingness to obtain).
- Strong interpersonal, verbal, and written communication skills.
- Excellent time management, planning, and organizational skills.
- Strong problem-solving skills and conflict resolution ability.
- Valid driver’s license and reliable vehicle.
- Proficient in basic computer skills and keyboarding.
- Experience in performance management is an asset.
- Accreditation Canada experience is a plus.
POSITION DETAILS:
- Job Type: Part-time
- Schedule: 22.5–25 hours per week, Monday to Friday. Flexible scheduling options: Mon, Tues, Thurs or Tues, Thurs, Fri. Regular weekday hours from 9 AM to 5 PM.
- Location: North Vancouver office (Lonsdale Avenue), with potential to work remotely as approved by management.
COMPENSATION AND BENEFITS:
- Pay: $39.50 - $42 per hour
