RESPONSIBILITIES MAY INCLUDE:
- Clinical Oversight: Oversees patient care delivery, providing feedback and ensuring it meets professional standards and protocols through support visits and audits.
- Initial Home Visit Assessments
- In Home Care Conferences with clients/ family and Government Funder.
- In Home Delegated Task training
- In Home PSW support visits
- In office PSW interview/ orientation
- In office Performance Development meetings
- Staff Management: Assist in recruiting and retaining PSWs, providing education and guidance, addressing conflicts and promoting effective communication.
- Professional Development: Acts as a mentor and educator for PSWs, providing feedback, sharing best practices and helping staff grow in their skills and knowledge
- Systems Improvement: Works to enhance healthcare delivery systems by enforcing policies and providing education to PSWs.
- Administrative Link: Serves as a crucial link between Government Funder and the front line staff.
QUALIFICATIONS:
- Must possess a Registered Practical Nursing (RPN)/ Registered Nurse (RN) Diploma and be in good standing with the CNO.
- Experience in Home Care is an asset
- Bilingualism is an asset
- Must Demonstrate effective leadership and communication skills
- Must be able to operate with cultural sensitivity and competence in their practice
- Must serve as a ethical model for staff.
