Reporting to the Community Support Coordinator, the Community Support Worker supports residents in achieving their individual goals, promoting independence, and enhancing their quality of life. This role involves facilitating and participating in recreational, leisure, fitness, and community inclusion activities that reflect residents' individual needs and interests.
The Community Support Worker provides life skills training, implements behaviour support strategies, and assists with rehabilitation, physiotherapy, and medication support plans developed by the interdisciplinary care team. Responsibilities also include transporting residents to appointments and community activities, maintaining accurate documentation, communicating professionally with residents and team members, and working collaboratively within a diverse team. Flexibility to work evenings and weekends is required.
You will also contribute to the development and implementation of individualized Care and Activity Plans, participate in departmental initiatives, and support the maintenance of outdoor spaces, including yard and garden care, to help create a safe, welcoming, and engaging environment for residents. Experience
· Relevant experience working with adults or children with a disability, preferably, adult survivors of a traumatic brain injury in a medical, community or residential setting.
· Six months practicum, volunteer, or work experience with challenged adults, preferably with survivors of traumatic brain injury.
Skills and Abilities:
A suitable candidate has the:
· Ability to function effectively within the constitution and Bylaws of The Cridge Centre for the Family.
· Ability to be an active listener and to problem-solve.
· Ability to respond effectively and calmly in all situations.
· Ability to depersonalize remarks communicated by residents.
· Ability to treat clients, staff and other professionals with dignity and respect at all times, which is an inalienable right.
Qualifications
· Activation Assistant Certificate/Community Support Worker Certificate or equivalent combination of training and experience.
· Certificate in relevant courses in the health care field may be accepted.
· Fire Safety Training.
· TB Test Clearance.
· Class 4 Driver’s License is considered a strong asset.
Certifications/Licensing:
· FoodSafe Certificate.
· Current First Aid/CPR Certificate.
· Criminal Record Check.
