JOB SUMMARY
The primary areas of responsibility include health promotion, health education, and community health development. The CHN uses independence and initiative to plan, organize, implement, and evaluate community health services, providing care in accordance with established nursing standards, departmental objectives, and all applicable Acts and Regulations. The CHN ensures that Public Health Nursing services are efficiently coordinated and accessible and actively participates as a team member in delivering quality programs and services.
REPORTING STRUCTURE
Reports to the Director of Health. Clinical oversight is provided by the Integrated Care Manager.
JOB DUTIES AND RESPONSIBILITIES
- Identifies needs and provides health services in homes, clinics, schools, and offices to individuals and groups
- Assesses care needs and educates individuals, groups, and the community through health programming sessions and events (e.g., diabetes, pre and post‑natal care, tuberculosis and other screenings)
- Provides health education and promotion services to support healthy lifestyle changes
- Provides education regarding protection against communicable diseases, including administering immunizations
- Plans, organizes, implements and evaluates community health services to promote wellness and prevent illness and disease
- Analyzes community health indicators, such as lab reports, communicable disease reports and statistics
- Provides treatments, support and follow‑up to ensure compliance
- Develops action plans for individuals, families, schools and the community for health emergencies and pandemic response
- Selects and implements services and activities that support positive health outcomes
- Advocates for and refers community members to appropriate agencies, medical providers and programs
- Supports coordination of client visits with physicians, hospitals, physiotherapy, occupational therapy, dental, optometry and audiology services
- Liaises with physicians, hospitals, teachers, social workers and other community service agencies
- Identifies health issues and needs by analyzing demographic, health and related data
- Provides services and establishes methods to measure effectiveness
- Maintains statistics (treatments, immunizations, etc.) to support health planning
- Coordinates Public Health Nursing services with community agencies
- Arranges and participates in joint home visits with other professionals
- Assists with planning and development of community health resources, including emergency and pandemic planning
- May be assigned emergency services duties during community emergencies
- Participates in community events as requested
- All persons employed by the Okanagan Indian Band may be required to assist the Band in providing emergency services. Duties assigned during an emergency may differ from regular duties

ADMINISTRATIVE DUTIES
- Develops community health work plans
- Creates health promotional materials and educational displays as part of the team
- Completes community health reporting, including Panorama
- Conducts home and office visits
- Attends professional development to maintain current knowledge and skills
- Attends community meetings and workshops relevant to the CHN role
- Promotes traditional and cultural health practices
- Prepares course materials and delivers sessions (e.g., expectant parents, parenting, weight management, elder health)
- Ensures all care complies with relevant policies, procedures and regulations
- Maintains a current, accurate, and confidential client reporting and filing system
- Maintains vaccine integrity
- Oversees biohazard management
- Participates in pandemic, emergency and community planning
- Reports concerns to the Director of Health
- Performs all duties in accordance with OKIB policies and within BCCNM scope of practice
- Performs other duties as required
WHO ARE WE LOOKING FOR
You have education, experience, or an equivalent combination may be considered. You are community-minded, detail-oriented, have a willingness to contribute to a team and an excellent communicator.
Education
- Bachelors degree in nursing (RN) from a recognized university
- Current, active registration with BC College of Nursing and Midwives (BCCNM)
- Current First Aid/CPR certificate with AED or willing to obtain
- Valid BC Driver’s license, minimum Class 5 and reliable vehicle
- Criminal Record Check with Vulnerable Sector Search
- Immunization Certification and knowledge/training and/or experience in Managing Communicable Disease within a community setting
Experience
- Minimum 2 years of experience working in a community setting
- Experience working with First Nations communities
- Strong team/client leadership, communication, problem solving and teamwork skills
- Experience with program planning, delivery and facilitation an asset
