We are seeking a dedicated and experienced Nurse Practitioner to join our healthcare team. Duties
- Conduct thorough patient assessments and develop individualized care plans
- Provide medical diagnoses and treatment for a diverse patient population, including pediatrics and seniors
- Administer medications and perform procedures such as phlebotomy when necessary
- Manage chronic conditions such as dementia and pain, ensuring effective symptom control
- Collaborate with multidisciplinary teams to coordinate case management and patient care strategies
- Document patient interactions accurately using electronic health record systems like Epic and Cerner
- Educate patients and families on health management, medication use, and preventive care
- Assist in laboratory procedures and interpret test results to inform treatment decisions
- Support nursing staff by providing clinical guidance and mentorship in various care settings
Qualifications
- Valid Nurse Practitioner licensure with current certification
- Experience working with children, seniors, or in acute care environments preferred
- Prior experience in nursing homes, hospitals, assisted living facilities, or home care settings is highly desirable
- Strong knowledge of medical terminology, medication administration, and patient care protocols
- Proficiency with electronic health records such as Epic or Cerner systems
- Excellent communication skills with the ability to collaborate effectively within healthcare teams
- Ability to handle multiple priorities in fast-paced environments while maintaining attention to detail
- Additional skills such as dementia care, laboratory experience, pain management, case management, or phlebotomy are advantageous
