CCH provides a full range of acute inpatient, outpatient and community mental health services to a catchment area of 100,000 people from the communities of Cornwall, the United Counties of Stormont, Dundas and Glengarry and to the Mohawk community of Akwesasne.

Reporting to the Manager of the Chronic Disease Management program, the incumbent will work as an active member of our interdisciplinary team. The incumbent will be responsible for providing quality nutritional care and education to patients of the Chronic Disease Management program. In addition, the incumbent will be required to contribute to program planning, implementation and evaluation. They will practice in accordance with the policies and procedures of the Hospital and the current Standards of Practice of the College of Dietitians of Ontario. The incumbent will embody a culture of care partner inclusion in the patient’s plan of care.
CCH seeks a Registered Dietitian who is a Certified Diabetes Educator (CDE) committed to providing high-quality education and healthcare services to people with diabetes. The incumbent will work as a multidisciplinary team member providing diabetes education counselling, community outreach and health promotion to individuals, families and the community. The successful candidate must be able to assist in any Chronic Disease Clinic as required.
